High urinary alpha 1 microglobulin indicates that glomerular injury, decreased tubular reabsorption capacity or tubulointerstitial nephritis may occur. 1. Glomerular injury: If the filtration function of glomerulus is damaged, it will not be able to filter alpha 1 microglobulin, resulting in alpha 1 microglobulin entering the urine. 2. Decreased reabsorption ability of renal tubules: when the reabsorption function of renal tubules is impaired, it will lead to the inability of α1-microglobulin to be broken down and absorbed, which will lead to the detection of elevated concentration of α1-microglobulin in urine. 3. Tubulointerstitial nephritis: a non-purulent inflammatory reaction dominated by renal interstitial lesions, the patient’s renal function is impaired, and the renal tubules are diseased, thus causing high α1-microglobulin values.
