The pain is mainly due to the catecholamines and metabolites produced by the adrenal glands that flow backwards into the spermatic vein after varicose veins are present, and the spermatic vein itself has no valves, so these chemical transmitters flow backwards into the testes through the spermatic vein, causing chemical irritation and thus the symptoms of hidden pain. Also, there are abundant venous anastomoses between the testes on both sides, so varicocele on one side can cause pain and discomfort in both testes. The patient usually needs a routine semen examination on top of this symptom, because varicocele can cause a decrease in sperm quality production in men, leading to infertility. If this problem occurs, the patient should undergo surgery in time.
