Advantages and disadvantages of three mainstream rhinoplasty methods

Rhinoplasty is one of the most commonly performed plastic surgery procedures for Oriental women. A standardized rhinoplasty involves embedding a shaped implant under the skin of the back of the nose to raise the bridge or tip of the nose, thus changing the shape of the nose to achieve the desired effect. According to the material, it can be divided into 3 types: autologous cartilage rhinoplasty, silicone implant rhinoplasty, and expandable rhinoplasty. Autologous cartilage rhinoplasty 1, adapted to the crowd: silicone prosthesis rejection reaction has occurred; done rhinoplasty due to a variety of reasons leading to failure; the nose needs to change the scope of the larger, with other materials can easily lead to complications; subjective request for their own cartilage materials for rhinoplasty. 2, nasal septum – nasal septum cartilage is the two nasal passages play a role in dividing the plate cartilage, its front and above each part of the retention, after the lower part of the whole removed, not only to maintain the integrity of the nasal form, but also to get the required materials, and take the material and rhinoplasty in the same field (refers to the range of vision during the operation), no need to It is really a “three birds with one stone” strategy. Advantages: Avoiding the reaction of alien tissue rejection, high safety, no need to make a second surgical area in other parts of the body, the total wound area of the human body is reduced. At the same time, the septum cartilage texture is hard, easy to shape both the round and curved nose, and can even by the way to treat the septum curvature or correction of nasal congestion and other diseases. 3, ear cartilage – ear cartilage is mainly used for nasal tip elevation surgery, with good compatibility and solid characteristics. Advantages: easy to carve and trim the model, easy to fix, no displacement and rejection. 4, rib cartilage – as another representative of transparent cartilage, rib cartilage has a large number of sources, reliable texture and safe extraction methods. It is the “ultimate weapon” for difficult congenital nasal deformities and acquired nasal deformities caused by trauma or surgery. Advantages: low absorption rate, will not penetrate the skin, non-vascular, easy to survive, will not be rejected, the use of a wide range. Silicone gel prosthesis rhinoplasty Silicone gel prosthesis is currently the most commonly used rhinoplasty material, for polymer silicone. 1.Adaptable people: subjective request for rhinoplasty candidates; low nasal bridge, hypertrophic nose, low nasal tip, hypertrophic or wide nose, nostrils toward the front; eyes, cheekbones, and poor frontal morphology caused by the poor visual effect of the nose; rhinoplasty failure in other ways. 2. Surgical Procedure: An incision of about 1mm is made on the inside of the nostril margin at the junction of the nasal column and the nasal wing, and a spacious cavity is separated under the nasal dorsal fascia or the periosteum of the nasal bone through the incision, and then the prosthesis is sculpted according to the requirements of the applicant and the doctor’s technique, or the sculpted prosthesis is implanted directly, and the skin is finally sutured back to its original position. Advantages: Silicone gel prosthesis is currently the longest clinically applied, the largest number and the widest range of rhinoplasty materials. Solid silicone has stable performance, little irritation, moderate texture, can be pre-plasticized, can be stored in the body for a long time without deformation, no adverse reactions to the correct use of good biocompatibility, and inexpensive, easy to deal with post-operative complications and unsatisfactory appearance. 3.Possible side effects: Unattractive nasal appearance – nose too high, nose too narrow, lack of curvature of frontal-nasal angle, hard nasal curve. Appearance of rejection reaction – nasal edema does not disappear for a long time, and there is yellow liquid outflow from the incision, skin redness. Translucent and exposed prosthesis after rhinoplasty – Translucent prosthesis is due to the enhanced reflection of the nose after rhinoplasty, which enables people to feel the presence of subcutaneous prosthesis. Silicone gel prosthesis exposure is manifested as the prosthesis penetrates the skin or mucous membrane of the nose and is exposed outside. Unstable or crooked nose – Postoperative instability of the prosthesis is manifested by the vague outline of the implanted prosthesis, and when touching the nose with the hand, the implanted nasal prosthesis is felt to be shaking under the skin. The nose is crooked because the axis of the bridge of the nose is not correct, the septum is crooked, the nasal column is crooked. Expanded body rhinoplasty Expanded body rhinoplasty is the use of expanded polytetrafluoroethylene as the prosthetic material. The material is compatible with the human body, non-toxic, non-carcinogenic, non-sensitizing, lifelong does not need to be replaced. 1, adapt to the crowd: low nasal bridge, low nasal tip or nasal hypertrophy and wide; eyes, cheekbones and forehead poor shape of the nose caused by the poor visual effect. 2.Surgical Procedure: Spot design – the incision of the spot design is marked out first, and then the injection needle dipped in melphalan solution is pierced into several key points of the design line, paying attention to not piercing too deep and causing bleeding. Incision – Before incision, lip clamps or suture ties can be used on both upper lips near the corners of the mouth to compress the labial artery, and the labial artery can also be pinched with the fingers to minimize bleeding. During incision, the candidate uses the middle finger of the left hand against the oral mucosa in the incision area and tenses or pulls the lip with the index finger and thumb, which makes the tissue tense and facilitates incision. Separation – In order to reduce the tension on the sutures of the incision, it is necessary to make a relaxing incision in the alveolar ridge on both sides and to make a blunt peeling of the periosteum, including the buccal soft tissues and the cleft side of the nose. For those with complete cleft lip, the attachment point of the outer corner of the nose to the underside of the inferior turbinate should be cut off, so that the outer foot of the nose can be freed sufficiently to facilitate better reset, and ultimately the nose can be made to achieve symmetry on both sides. Suture – first suture the bottom of the nose to make the nose reset, and then suture the lip mucosa and orbicularis oris muscle, the latter should achieve functional reset. Advantages: the texture and feel of the bentonite material is very close to the normal nasal tissue, beautiful and natural after surgery, it is difficult to detect; with a special microporous structure, the body tissue cells and blood vessels can be grown into them to form a tissue connection; do not move, do not deform, and the nasal tissue as one, and it is impossible to penetrate out of the skin; the bentonite material does not have the “transparency” of the silica gel. “No transparent feeling of silicone; light weight of the material, no feeling of falling. Disadvantages: Because of the softer and not strong enough, the effect is not ideal in the surgery that needs to raise the tip of the nose; it is not easy to be processed and sculpted into shape, the technical level of the doctor’s requirements are very high, and if not properly operated, it is easy to cause side effects; once the effect of the surgery is not ideal, it is difficult to take out the prosthesis, and only an experienced doctor can take it out in a complete way.
