Cholangiocarcinoma occurring in the middle segment is relatively well treated. According to the location of tumor, bile duct cancer can be divided into upper, middle and lower bile duct cancer, and the scope of its surgical resection is also different.
1. Upper bile duct cancer: located in the area above the opening of the right and left hepatic ducts to the cystic duct. If the tumor is confined to the bile duct, the gallbladder and extrahepatic bile duct need to be removed, and then choledocho-jejunostomy is performed; if the tumor invades the liver, partial hepatectomy or hemihepatectomy will be added.
2. Middle cholangiocarcinoma: it is located between the opening of gallbladder duct and the upper edge of duodenum. Only the tumor and the bile duct more than 0.5 cm from the edge of the tumor need to be removed.
3. Lower bile duct cancer: located between the upper edge of duodenum and duodenal papilla. Since the lower part of bile duct is closely connected with duodenum and pancreas, pancreaticoduodenectomy is necessary.
Therefore, the surgery of middle bile duct cancer is relatively better, but the treatment of bile duct cancer also needs to consider various factors such as pathological type of tumor, staging, metastasis and so on, and should be carried out by professional doctors.
