Treatment goals for chronic hepatitis B Treatment goals: To maximize long-term inhibition of hepatitis B virus (HBV) replication, reduce hepatocyte inflammatory necrosis and liver fibrosis, achieve delay and reduce the occurrence of liver failure, cirrhosis decompensation, hepatocellular carcinoma (HCC) and other complications, thus improving quality of life and prolonging survival time. During treatment, clinical cure of chronic hepatitis B, i.e., sustained virological response after cessation of treatment, disappearance of surface antigen (HBsAg) with alanine aminotransferase (ALT) reversion and improvement of liver histology, should be pursued as much as possible for some suitable patients. Treatment endpoints: (1) Desirable endpoints: E antigen (HBeAg)-positive versus HBeAg-negative patients with durable HBsAg disappearance with or without HBsAg serological conversion after drug discontinuation. (2) Satisfactory endpoints: HBeAg-positive patients, obtaining sustained virological response and ALT normalization with HBeAg serological conversion after discontinuation; HBeAg-negative patients, obtaining sustained virological response and ALT normalization after discontinuation. (3) Essential endpoints: if sustained response after drug discontinuation cannot be obtained, virological response is maintained for a long time during antiviral therapy (undetectable HBV-DNA). The “gold medal”, “silver medal”, “bronze medal”, and “iron medal” of chronic hepatitis B treatment are summarized in layman’s terms by the author below (1) “Gold medal”, that is, the HBsAg turns negative, if the S antibody (HBsAb) turns positive, it is “platinum medal”. (2) “Silver”: for patients with “major triplet”, become “minor triplet”, is the silver medal. (3) “Bronze”: a relatively long time hepatitis B virus DNA is below the detection value. (4) “Iron”: “transaminase” ALT is normalized. In summary, the new guidelines call for the pursuit of a “gold medal”, i.e., clinical cure, whenever possible. But the pursuit of “gold medal” is not easy, often need to get the “silver”, “bronze”, “iron”. So don’t be discouraged and proceed step by step. In any case, try to pursue to the age of 70 or 80, still do not have liver cancer, liver cirrhosis ascites, etc.. And live a happy life.
