In 1978, the World Health Organization (WHO) defined intestinal epithelial metaplasia (intestinal metaplasia or enteroplasia) and heterogeneous hyperplasia (or atypical hyperplasia) as precancerous lesions of the stomach. Some domestic scholars have followed up patients with chronic gastritis with intestinal epithelial hyperplasia and/or heterogeneous hyperplasia for 2-10 years, and found that the cancer rate ranged from 1.8% to 36.4% with different degrees of lesions. From the results of recent years, it is clear that among them, type III intestinal epithelial hyperplasia and moderate and severe heterogeneous hyperplasia are more closely related to gastric cancer and are truly precancerous lesions in the stomach. The evolution of gastric precancerous lesions has a certain pattern, from normal gastric mucosa → superficial gastritis → atrophic gastritis → intestinal epithelial hyperplasia → heterotypic hyperplasia → intestinal gastric cancer, and this pathogenesis pattern has been widely accepted now. Gastric mucosal intestinal epithelial hyperplasia and heterotypic hyperplasia can be classified as mild, moderate or severe according to their degrees, among which severe heterotypic hyperplasia is difficult to distinguish from early gastric cancer in terms of pathology and surgery is mostly advocated. The former can be divided into type I (complete small intestinal epithelial metaplasia), type II (incomplete small intestinal epithelial metaplasia) and type III (incomplete large intestinal epithelial metaplasia or colonic epithelial metaplasia), while the latter can be divided into adenoma, crypt type, regenerative type, heterogeneous cystic dilatation type and spherical metaplasia. Modern medicine once held the view that precancerous lesions of the stomach could not be reversed, but now the number of people holding this view has gradually decreased, but no effective treatment has been identified. Chinese medicine has a long history and a unique theoretical system. Its holistic concept, evidence-based treatment and the advantages of adapting to individual, time and place make it unique in the prevention and treatment of many diseases with remarkable efficacy. A large number of clinical practices have shown that the use of TCM theory in the treatment of gastric precancerous lesions can achieve better efficacy and is worthy of further study. In the treatment of gastric precancerous lesions, TCM mostly adopts the method of differentiation and typing, that is, according to the changes of symptoms, signs and symptoms, tongue and pulse, patients are divided into different types such as liver-stomach disharmony, spleen-stomach damp-heat, stomach complex blood stasis, stomach-yin deficiency and spleen-stomach weakness, and are treated with different prescriptions such as dredging the liver and stomach, clearing heat and dampness, activating blood circulation and removing blood stasis, benefiting the stomach and nourishing yin, and strengthening the spleen and stomach. Clinically, on the basis of typing treatment, we can also adopt the basic fixed formula plus or minus treatment according to the main etiological factors of gastric precancerous lesions. In a comparative study of 348 cases of general chronic gastritis and gastric precancerous lesions, we found that gastric precancerous lesions are characterized by a mixture of deficiency and reality, with the main pathogenic mechanism being Qi deficiency and Qi stagnation, and at the same time intermingled with changes such as Damp-Heat and Blood stasis. Therefore, according to the traditional theory of Chinese medicine, “treating the middle jiao like a balance is not peaceful and unsettling”, we formulated the basic formula “Legastric Decoction”, which has the effects of strengthening the spleen and regulating qi, clearing heat and detoxifying toxins, and activating blood circulation and removing blood stasis, and it has been applied clinically with good results. Since the treatment process of gastric precancerous lesions is long, usually more than 6 months, many patients hope to be treated with Chinese medicine. It should be said that this request is understandable and can be achieved, specifically, there are three ways as follows: (1) due to the complicated condition of gastric precancerous lesions, it is better to treat them with tonics plus and minus at the early stage of treatment (within 3-6 months), and after the main symptoms are controlled, appropriate proprietary Chinese medicine will be used to consolidate the treatment; (2) the hospital will make the empirical prescription into proprietary Chinese medicine for in-hospital use (concentrated decoction and punch are available), together with some temporary (3) According to the patient’s specific condition, several kinds of commercially available proprietary Chinese medicines can be used in combination. We have treated some patients with mild to moderate gastric mucosal heterogeneous hyperplasia or moderate to severe intestinal hyperplasia with the combination of nourishing stomach punch, gas stagnation and gastroparesis punch and gastric rejuvenation tablets, and obtained good results that intestinal hyperplasia and heterogeneous hyperplasia disappeared (confirmed by two consecutive gastroscopic examinations). Improper diet, emotional disorders and irregular life are important causes and aggravating factors of gastric precancerous lesions, therefore, attention should be paid to the regulation of these aspects at the same time of treatment. We should avoid smoking, alcohol, coarse, hard and spicy stimulating foods, and be careful with drugs that are harmful to gastric mucosa, such as aspirin, etc. We should maintain a regular, quantitative, hot and cold appropriate and nutritious diet; control the occurrence of bad emotions such as anger, grief and sadness, and adjust life goals to maintain a good state of mind; combine work and rest, relaxation and relaxation to ensure a normal life pattern. As the saying goes, chronic diseases are “three parts treatment, seven parts recuperation”, which is very reasonable. With proper treatment and active cooperation, precancerous lesions of stomach can be controlled or even cured.
