The first consideration for a roundish nonenhancing nodular shadow in the kidney is a renal cyst.
Renal cysts often present as round-like nonenhancing nodular shadows in the renal parenchyma or on the renal surface. In most patients, the renal cysts are simple renal cysts. Simple renal cysts are benign lesions that do not require treatment or management for small, asymptomatic renal cysts.
However, with the prolongation of time, renal cysts may become larger. If the diameter of renal cysts is larger than 5cm, or if there is suspected hemorrhage, malignancy or compression of normal renal parenchyma when ultrasound and CT examination are performed, and the symptoms of lumbar acidity and lumbar pain appear, treatment is needed.
For larger cysts, puncture aspiration and injection of sclerosing agent such as anhydrous ethanol may be considered to prevent recurrence. Cystectomy or even nephrectomy should be considered in patients with large size cysts, malignant tendency or recurrence after puncture.
