What is the method of resetting the temporomandibular joint?

TMJ repositioning methods include intraoral and extraoral methods. 1. Intraoral method: The patient is asked to sit on an ordinary chair with the head close to the wall. The position of the mandibular tooth surface should be lower than the level of the elbow joint when the operator’s arms are hanging down. The operator stands in front of the patient, both thumbs wrapped in gauze into the patient’s mouth, covering the jaw plane of the posterior mandibular teeth, and the remaining four fingers hold the angle of the mandible and the mandibular body, to the lower back of the overall displacement, and then the upper back of the reset. Sometimes a crisp popping sound can be heard at the moment of resetting. When it is difficult to reset both sides at the same time, one side can be reset immediately after the other. 2. Extraoral method: The position of the patient and the operator is the same as the intraoral method. When resetting, the operator puts both thumbs on the anterior edge of the condyle protruding below the zygomatic arch on both sides of the patient, i.e., at the Xiaguan acupoint, and then forcefully squeezes the condyle downward and backward. At this time, the patient feels soreness and numbness in the lower jaw; the operator simultaneously holds the angle of the two lower jaws with the middle fingers of both hands, holds the lower edge of the body of the lower jaw with the ring finger and the little finger, and the fingers work together to make the angle of the lower jaw and the body of the lower jaw pushed upward and forward, at this time, the condylar process descends and slides backward into the articular fossa and is reset. When the mandible is reset, due to the reflexive contraction of the masticatory muscles, the upper and lower teeth are closed tightly and the operator’s thumb may be bitten. Therefore, when resetting the jaw by the intraoral method, the operator’s thumb should be slid to the buccal vestibule quickly to avoid biting. If discomfort occurs, please consult the hospital in time to avoid delay.
