Abstract: The patient had recently been staying up late studying and overusing his eyes, resulting in bumps on the eyes, and his family came to our hospital because they were concerned about the patient’s health. After taking the patient’s medical history and giving relevant examinations, the patient was initially diagnosed with a blepharosphenoid cyst and was admitted to our hospital for treatment after informing the patient and his family. The patient has been treated conservatively and her symptoms have been cured without recurrence.
Basic information】Female, 20 years old
Type of disease】Eyelid cyst (blepharocyst)
Hospital】Shanghai Sixth People’s Hospital
Date of Consultation】March 2022
Treatment plan】Hot compresses + topical medication (compound zinc sulfate drops, erythromycin eye ointment) + intracapsular injection (dexamethasone injection)
Treatment period】3 weeks of home treatment, half-month follow-up
Treatment effect] The patient’s symptoms were cured
I. Initial consultation
After the family brought the patient to the clinic, they complained that the lump in the patient’s eye had not subsided for 2 weeks. On closer examination of the patient’s eyes, it was found that the entire upper eyelid was red and swollen with small bumps, which were obvious when the eyes were closed. Examination under the slit lamp showed that the patient had lipid plugs visible on the right eyelid margin, some yellow crusts at the roots of the eyelashes, lid conjunctiva with congestion, and no intraocular abnormalities. We asked the patient about his recent eye use and whether he liked to rub his eyes. We learned that the patient had recently stayed up late studying for an exam.
II. Treatment history
The patient and his family were informed of his condition, and after communication, conservative treatment was chosen, including the application of hot towels to the eyelid cysts.
In addition, patients are advised to pay attention to some eye habits in daily life, pay attention to eye hygiene, keep eyes clean, and do not wipe eyes with dirty hands or towels. Pay attention to good eye habits, avoid staying up late and overuse of eyes, etc.
III. Treatment effect
After 1 week of treatment, the patient came for a follow-up examination and saw a slight change in the upper eyelid cyst in the right eye, but the swelling was still more pronounced, so a small amount of dexamethasone injection was given into the cyst to promote absorption. After another 2 weeks, the patient came back for a follow-up visit, and by this time, both eyelids were seen to be symmetrical, with no significant bulging or other abnormal symptoms in the right eye, so the patient was instructed to stop the medication.
The patient’s symptoms, such as bulging of the right eye with redness and congestion, were cured compared to before the visit by actively cooperating with relevant examinations and timely treatment. At present, the patient’s eyelids are basically symmetrical, and no differences can be seen in both open and closed eyes, and there are no cysts on the eyelids or conjunctival surface, and the conjunctival surface is intact.
IV. Notes
I was relieved to see the patient’s symptoms gradually reduced, but since the disease is often prone to recurrence, the patient needs to be reminded to pay attention to the following points in daily life.
1. Patients are advised to develop good eye habits, including avoiding staying up late and looking at cell phones for a long time, and it is recommended to maintain good sleep, which can help to relieve the disease. In addition, patients need to pay attention to avoid rubbing their eyes to keep them clean.
2. Diet needs to be light and balanced, while strengthening nutrition and paying attention to balance, avoiding intake of greasy and spicy stimulating foods such as chili, onion, ginger, garlic, hot pot, etc.
3.Patients are recommended to exercise appropriately, pay attention to exercise, enhance physical fitness, and breathe more fresh air, etc., all of which can properly relieve eye strain.
V. Personal insight
In clinical practice, lid gland cysts are a common disease in ophthalmology, usually due to obstruction of the discharge of lid gland secretions, and are influenced by a variety of factors, such as poor lifestyle habits, and are often seen in adolescents or middle-aged adults.
It is important to note that blepharospasm cysts are less likely to heal on their own, especially if they are more severely blocked, and if they are not treated in a timely manner, they may cause greater harm to the patient, including the tendency for local tissue to degenerate and even form granulomas, so it is important to seek treatment at the hospital when they first appear. Fortunately, the patient’s cyst was small and was found in time, and the patient was cured in time by applying heat and using medication.
