Early stage pancreatic cancer usually does not present with pain. If a patient with pancreatic cancer develops pain, it often indicates that the pancreatic cancer is not early stage. Pancreatic cancer tumors can cause obstruction of the pancreatic duct or bile duct, thus increasing the pressure in the pancreatic duct or bile duct, or the pancreatic tumor invades the pancreatic envelope, which can lead to abdominal pain or abdominal discomfort. If the tumor invades the nerve plexus in the abdominal cavity, there is often persistent and severe low back pain. Patients tend to curl up in a position to relieve the pain because of the pain, and such pain is more pronounced at night time. Also some tumors can cause chronic inflammation of the surrounding pancreatic tissue, which may also cause pain.
