The highest point of the iliac crest is palpated by hand palpation. To determine the highest point of the iliac crest, first place your hand on the posterior aspect of your buttocks and palpate forward and outward, with the most prominent bulge being the highest point of the iliac crest on either side. For thinner individuals, the most obvious bulges can be seen on the sides of the abdomen and upper thighs as the highest points of the iliac crests on each side. You can also look for the lumbar vertebrae, counting down from the first lumbar vertebrae, and when you touch between the fourth and fifth lumbar vertebrae, parallel to the sides, you will see the highest point of the iliac crest on both sides. The location of the highest iliac crests on both sides is also the location where we usually tie our belts, and it is an important location for bone marrow puncture and bone grafting in the clinic.
