Cirrhosis spitting blood may cause hepatic coma (hepatic encephalopathy), cirrhosis spitting blood main reason for acute gastric mucosal lesion bleeding and portal hypertension gastric fundus hemorrhage.
1. Acute gastric mucosal lesion bleeding: cirrhosis patients’ gastric mucosal microcirculation obstacle due to portal hypertension or impaired liver function increase the retention of toxic substances in the body, destroying the gastric mucosal barrier, thus causing ulceration or bleeding in the upper gastrointestinal tract. In this case, the amount of bleeding is small, and the occurrence of hepatic encephalopathy is more related to the accumulation of toxins, and a small amount of vomiting blood will not directly trigger hepatic encephalopathy.
2. Portal hypertension gastric fundus hemorrhage: at the end stage of cirrhosis, the pressure of portal vein increases abnormally, resulting in abnormal metabolic disorders of portal vein-corporeal circulation shunt and increase of toxin induced hepatic encephalopathy, coupled with esophageal and gastric fundus varices in the lateral branch of portal vein, it is very easy to rupture and bleed, triggering hemorrhagic shock, aggravating the incidence of hepatic encephalopathy and the severity of its development, and it is important to immediately rescue the patient if it occurs. Resuscitation should be done immediately if it occurs.
It is impossible to tell whether hepatic encephalopathy will be caused by the symptoms of cirrhosis alone, but once there are bleeding symptoms, we should send the patient to the doctor in time to avoid delaying the condition.
