Gastroesophageal reflux disease (GERD) refers to a disease caused by the reflux of stomach and duodenal contents into the esophagus, which can lead to inflammation, erosion, ulceration and even cancer of the esophageal mucosa. Its typical symptoms are acid reflux, regurgitation, heartburn, belching, bloating and other digestive symptoms, but also cough, coughing, tightness in the throat, wheezing, aspiration pneumonia, non-cardiogenic chest pain, reflux pharyngitis, rhinitis, otitis media and other extra-esophageal manifestations, which seriously affect the quality of life of patients. It is important to note that many patients who have been misdiagnosed as having “pharyngitis” for a long time are actually suffering from GERD. Gastroesophageal reflux disease is a common disease, with about 7-15% of the Western population having GERD symptoms. In China, in 1999, the flow survey in Beijing and Shanghai showed that the incidence of symptoms was 8.97%, and there is a trend of increasing year by year. GERD symptoms can be divided into: (1) typical symptoms: heartburn, acid reflux, regurgitation; (2) atypical symptoms: chest pain, abdominal pain, nausea, belching, early satiety, abdominal distension; (3) extra-esophageal symptoms: including recurrent oral ulcers, dental caries, bad breath, excessive saliva, chronic pharyngitis, foreign body sensation or blockage in the pharynx, hoarseness, sudden deafness, chronic cough, coughing, dyspnea, pneumonia, bronchial asthma, etc. GERD with pharyngitis as the main symptom is part of the extra-esophageal symptoms. Most of the GERD with pharyngitis as the main symptom does not have obvious acid reflux symptoms, but belching or hiccups may appear. According to some scholars, the symptoms of pharyngitis are: foreign body sensation in the pharynx, sore throat, hoarseness, and dry cough, with the incidence of 32%, 20%, 16%, and 11%, respectively. Diagnosis is based on the diagnostic criteria of chronic laryngitis, such as pharyngeal discomfort, foreign body sensation, sore throat, hoarseness, and dry cough; local signs: pharyngeal congestion, lymphatic follicle hyperplasia in the posterior pharyngeal wall; or edema in the spoon area seen under fiberoptic laryngoscopy. Diagnosis of reflux esophagitis was confirmed by electrogastroscopy; or symptoms were relieved after 4 weeks of diagnostic oral medication for GERD. Gastroesophageal reflux disease with pharyngitis as the main manifestation is one of the common diseases in otorhinolaryngology. Many patients with pharyngitis have taken numerous anti-inflammatory drugs and medications for pharyngitis without any effect, which seriously affects the health and life of patients. Nowadays, this disease has not attracted the attention of non-specialists, and many patients with gastroesophageal reflux disease are misdiagnosed with pharyngitis.
