Recognition and treatment of carpal tunnel syndrome

Carpal tunnel syndrome is a condition in which compression of the long flexor tendon of the thumb, superficial flexor tendon of the finger, deep flexor tendon of the finger and median nerve after squeezing or reducing the capacity of the carpal tunnel causes pain, numbness and muscle weakness in the thumb, index finger and middle finger. In daily life, the keyboard, especially the mouse is our most common “carpal tunnel killer”. With the increasing number of people driving, the steering wheel has also become a major “carpal tunnel killer”. Other professions that use their hands frequently, such as musicians, teachers, editors and journalists, architects, assemblers, etc., are all at risk of suffering from carpal tunnel syndrome. Therefore, proper recognition and effective prevention are the top priorities in the treatment of this disease. Liu Limin, Department of Orthopedics, Xuanwu Hospital, Capital Medical University The so-called carpal tunnel is a bony fibrous canal, with the radial side being the navicular bone and most of the angular bones; the ulnar side being the pea bone and hook bone; the dorsal side being the skull, navicular bone and small polygonal bone; and the palmar side being the transverse carpal ligament. This condition has rapidly become an increasingly common disease of modern civilization, and if left untreated for a long time, it may lead to nerve damage and hand muscle atrophy. The disease manifests itself initially as edema and congestion of the median nerve, gradually causing fibrosis within the nerve due to compressive ischemia, compression of the nerve axons and loss of the myelin sheath, and eventually irreversible changes as the nerve tissue turns fibrous and its intra-neural tube disappears and is replaced by collagenous tissue. Muscle tissues will also show local atrophy when they are reduced or lose nerve nutrition. Therefore, you need to pay more attention when you notice the following characteristics in both hands, including: feeling weakness in one or both hands, numbness or irritating stiffness in the fingers or palms, wrist pain, and uncomfortable and painful extension of the thumb. For the prevention of the disease, should focus on creating a healthy working environment and the correct operation of self-protection. Such as personal seats to the appropriate height, so that people sitting with enough space to stretch into the legs and feet; do not sit or stand for too long; sitting back should be straight and close to the back of the chair, and do not cross the feet, so as not to affect blood circulation. Typing computer keyboard should be facing you, if the side, may lead to excessive wrist bending tight; keyboard placement height and parallel distance from the body should be adjusted to a typing feel comfortable position; at the same time, every 30 minutes of operation, should pause for a while, so that hands and eyes for appropriate relaxation or rest. Don’t forget to protect your body. As for the treatment of this disease, although local closure can temporarily relieve the symptoms, but more can not be lifted at all. The cause of the disease should be identified to take the appropriate treatment measures, such as anti-rheumatic treatment or surgical release. Patients themselves can do local hot compress or massage, massage hard at the midpoint of the transverse palm, 15 minutes each time, 3 times a day, often can receive good results. Generally, the treatment methods used are just oral blood circulation or pain relieving drugs, acupuncture, tui-na, etc. There are also some auxiliary treatments such as heat therapy, i.e. hydrotherapy, wax therapy, ultrasound or hot compress, followed by electrotherapy to relieve pain, and more experienced physical therapists will also guide patients to do home exercises, but the effect is still very limited. Of course, if severe neurological symptoms occur, surgical treatment can be used to reduce the pressure in the carpal tunnel and relieve the compression if necessary.
