We often encounter patients, mostly young boys or girls, who ask, “Can I have my nails reshaped? I’ve been to so many hospitals.” Why do other hospitals not perform or rarely perform nail surgery? Because doctors who are accustomed to operating with large incisions rarely pay attention to such a small thing, the nail is so small that it is ignored. In fact, the nail plays a very important role in the hand, from a functional point of view: it protects the end of the finger, assists in holding objects, increases stability and accomplishes some very important functions in life, such as clamping very small things, or completing actions such as digging objects. From the aesthetic point of view: as the saying goes: the hand is the second face of a woman. Imagine if the slender hand is one or more deformed nails, it will make the beauty greatly reduced. As the awareness of beauty has increased in recent years, a significant number of male patients have requested plastic surgery for deformed nails. The main reasons for the deformed nails are: First, congenital deformities, the number of such patients is not small, mainly due to genetics or the mother’s pregnancy stimulated by external factors, which affect the development of fingers and nails and lead to deformities, such as short and wide thumb nail deformities and congenital hook nail deformities. Secondly: after trauma, there are many patients of this type, mainly young male and female factory workers who suffer from nail deformities due to hand trauma, often accompanied by previous fractures or crush injuries to the soft tissues of the skin, etc., which are also quite difficult to repair. There are also some patients who suffer from accidental injuries. Thirdly: infectious. The most common one is nail fungus, which is caused by compression of the nail into the soft tissues on both sides and leads to breakage and infection, most commonly in the thumb. In addition, there are fungal infections, which are often treated first with antifungal treatment in dermatology. In addition, there are also tumorigenic and so on, but they are not common and will not be described in detail. Since I entered the door of hand surgery more than ten years ago, I have been interested in nail deformity revision, but there are no such monographs at home and abroad, and most domestic doctors tend to pay insufficient attention to it. I remember five or six years ago, I accidentally found nail surgery (nail surgery) in the foreign language department of the school library, and I found that foreigners have done a lot of detailed work, and there are many things that we should learn from. Nail surgery usually has two ideas, one is the nail deformity due to the unhealthy surrounding tissues, such as the hook nail deformity caused by the finger scar, which is to repair the unhealthy tissues around the nail, so that the nail and nail bed is well reset and correct the deformity. There are also soft tissue deformities on the dorsal side of the finger (medically known as epithelium), which often lead to nail deformity due to scar pulling, and again the appearance of the nail can be improved by repairing the scar tissue through flap or implant surgery. Secondly, the soft tissues around the nail are normal, it is the soft tissue underneath the nail – the nail bed – that is faulty, and a problem with any part of the nail bed can lead to a deformity of the nail. Due to the special nature of the nail bed and the non-renewable nature of the nail bed, we generally solve the problem by transplanting the nail bed, which is usually taken from the toe, because the nail bed of the toe is richer, and removing a small part of it has little effect on the appearance. Although modern medicine has made great progress, there are many things to respect the reality, science fiction and advertising that do not respect the facts often exaggerate the effect and mislead people. Patiently communicate with your doctor to find out what is achievable and what is currently unattainable is very crucial.
