Abstract: Patient Li, 70 years old, suddenly developed symptoms of slurred speech and left-sided limb immobility when he got up at night. His family rushed him to the hospital and perfected cranial magnetic resonance examination suggesting cerebral infarction, which was diagnosed as ischemic stroke, or ischemic stroke. After admission, he was given active treatment with intravenous thrombolysis, and the revascularization treatment was effective, with significant reduction of adverse manifestations.

I. Initial consultation
In December 2021, the patient’s speech was slurred, his tongue was big, and his limbs were not moving well when he woke up at night. After arriving at the hospital, the patient’s emergency cranial CT examination was perfected, and the results suggested cerebral infarction and a clear diagnosis of ischemic stroke. Because the patient was sent to the hospital in time, thrombolytic treatment could be performed. After communicating with the patient and his family, the patient expressed understanding and willingness to actively cooperate with the treatment.
II. Treatment history
The patient was considered to be suffering from ischemic stroke, and the patient’s condition was stabilized without further aggravation by urgent investigations of blood routine, coagulation function, electrocardiogram and chest X-ray, which excluded contraindications to thrombolysis. After the patient was admitted to the hospital, cranial MRI and angiography were completed to assess the cerebrovascular condition, suggesting multiple sclerosis of the cerebral arteries, and the patient was given hemosiderin injection, olanzapine injection, edaravone injection, aspirin enteric tablets and atorvastatin calcium tablets. The patient was also asked to improve daily living habits, including bed rest and light diet, etc. He was discharged after 14 days and came to the hospital for a follow-up visit in 1 month, and was given rehabilitation training after evaluation.
III. Treatment effect
During the hospitalization, the patient’s condition did not deteriorate further, and the symptoms of limb immobility and slurred speech were relieved. At 14 days of hospitalization, the patient’s muscle strength on the paralyzed side recovered from level 1 to level 4, and his speech was slightly slurred, so he was discharged from the hospital. Before discharge, the patient was advised to adhere to rehabilitation training, regularly monitor high-risk factors such as hypertension and hyperglycemia, take medication on time, and have regular neurology follow-ups, which the patient understood.
IV. Notes

V. Personal insight
Once symptoms such as crooked nose and eyes, hemiplegia, unclear speech, etc. occur as in this case, you should actively go to the hospital for early treatment, which is more effective and can avoid the occurrence of stroke sequelae. Once post-stroke sequelae appear, long-term rehabilitation training should be adhered to. Six months after stroke is an effective rehabilitation period, and active treatment helps to improve the sequelae.
