Overview of rhinoplasty problems

  Rhinoplasty is one of the most dramatic plastic surgery procedures to improve appearance and is one of the most popular procedures among Caucasians, second only to blepharoplasty among Asians.  The nose is located in the middle of the five senses and protrudes from the face, making it the most prominent organ. Due to their skeletal structure, Oriental people have a flat face, except for the nose, which is more prominent. Therefore, its size, height and width have a great impact on the overall effect of the face. If the nose is low and flat, it will make the face look more wide and flat; if the nose is high, it will make the facial contour more three-dimensional.  In terms of morphological structure, the nose is the most delicate and complex of the five senses. Its three-dimensional cone structure, with two nostrils at the bottom, can be divided into more than twenty parts anatomically, and changes in any one part may cause great differences in appearance. Therefore, both in medicine and in folklore, there are many kinds of classifications for the shape of the nose. But in general, the Chinese have the majority of low and flat noses, especially in the south.  While Westerners are more likely to undergo “nose reduction”, for Easterners, “rhinoplasty” is the most popular and one of the most effective procedures.  Rhinoplasty is commonly known as “nose job” and the meaning is obvious. The main concern of patients is the material and the effect of this surgery.  After a long period of clinical observation by plastic surgeons, the most ideal material is considered to be medical silicone rubber, which is available in a wide variety of types, including segmented and one-piece. It is extremely stable and does not react with other substances; it is non-toxic, carcinogenic, and teratogenic; it is soft and easy to sculpt and plasticize; its biggest advantage is that it can be easily and completely removed after surgery when the patient thinks the appearance is not ideal, without leaving any defects. At the same time, it is also the cheapest. Therefore, the majority of patients who undergo rhinoplasty choose silicone rubber.  At the same time, there are various other materials being used for rhinoplasty, such as tumescent (polytetrafluoroethylene), injectable materials (hyaluronic acid, collagen). However, they exist for a short time and are more expensive; however, their most fatal drawback is that they are not easily removed intact after surgery, which makes plastic surgeons quite a headache. In particular, some injectable liquid materials are very difficult to remove completely when they are injected and fused with the surrounding soft tissues.  For patients, it is a bit overly specialized to let them choose their own materials. In fact, the most important thing for her/him to do is to choose a professional plastic surgery hospital, then communicate well with the doctor and find the one you trust the most to undergo the surgery.  Communication is very important, because everyone has their own different conditions and will have different requirements; at the same time, even professional plastic surgeons, due to different experiences, their cultural literacy and aesthetic standards will be different. Therefore, do not be disturbed by any outside interference, fully communicate with the doctor, and if enough trust can be generated between each other, then it is not far from a successful surgery.  For most Chinese women, in our experience, a nose that is not too high, not too straight, with a slightly softer curve and a natural appearance is the most acceptable. This is very much in line with Chinese aesthetics and the “middle of the road” cultural view. Of course, any change in nose shape is based on the original nose shape of the individual patient, and it is not necessary or possible to completely abandon the original nose shape and create a very different nose shape through rhinoplasty.  As for the specific surgical procedure, what patients need to know is relatively simple: female patients should avoid menstruation during surgery; the surgical incision is usually located on the inner edge of one side of the nostril, which should be avoided when washing the face after surgery; swelling of the nose will occur after surgery, which is more obvious 2-3 days after surgery and can basically subside after a week; sutures are usually removed 6 days after surgery.  Rhinoplasty for oriental women is really an invention that is many times more brilliant than lipstick, perfume and stockings. There is no pain, very little cost, and almost no danger in exchange for a significant improvement in appearance, a boost in temperament, and a confidence that comes from within.
