Sleeplessness is a type of disease characterized by frequent failure to get normal sleep, commonly known as “insomnia”. Patients mostly show difficulty in falling asleep; or waking up easily and not being able to sleep again after waking up; or not sleeping soundly and dreaming a lot. The medical doctors of all generations have discussed insomnia, and the treatment methods are abundant. The pathogenesis of sleeplessness can be summarized as “Yang does not enter Yin, and Yin and Yang are out of intercourse”, including Yin deficiency not being able to absorb Yang, or Yang Sheng not being able to enter Yin. The clinical treatment is mostly based on qi-depression and fire, disturbing the heart and mind; disharmony of stomach qi and internal disturbance of phlegm-heat; yin-deficiency and fire, lack of intercourse between heart and kidney; thought and labor, internal injury to heart and spleen; deficiency of heart and gallbladder qi, easy to be frightened and good at fear; stagnation of blood, blood does not nourish the mind. I had the honor to study for many years with Professor and Chief Physician Guo Lizhong, Director of the Institute of Acute Medical Problems of Nanjing University of Traditional Chinese Medicine, and witnessed that the teacher initially treated patients with insomnia according to the traditional method of identifying and treating evidence, but the effect was not long-lasting, and even individual patients were still not effective and suffered from insomnia. Mr. Guo then studied these cases and found that in addition to difficulty in sleeping, their clinical manifestations were mostly accompanied by cold feet, even though their mouths were dry and thirsty, they did not drink much water or were thirsty for hot drinks, and their tongues were light and fat with a watery coating, and even individual patients had obvious signs of Yang deficiency such as fear of cold and chill, and combined with the provisions of “Treatise on Typhoid Fever”, “Bai Tong Tang” and “Tong Yue Si Wei Si Wei Tang”, he realized that “Yang does not enter Yin, Yin and Yang are out of joint “In the case of “yang deficiency, yin-cold, yin-yang is not intersecting”, I took the treatment of warming up the floating yang and guiding the dragon into the sea, and it worked like a rafter. The following is a description of the treatment for the benefit of readers. 1.Basic formula: 60g of Pseudostellaria (first decoction for two hours), 30g of ginger, 5g of roasted licorice, 15g of Atractylodes Macrocephala, 15g of sand, 15g of cypress, 20g of wood butterfly, 30g of raw dragon peony (first decoction), 30g of raw dragon tooth (first decoction), 20g of fried jujube, 15g of Zhu Fu Shen, 30g of Gui Zhi. If the patient has obvious night sweating, replace raw dragon peony with calcined dragon peony; if dizziness and tinnitus are obvious, add 30g of magnet. If dizziness and tinnitus are obvious, add magnet 30g (first decoction); if insomnia is stubborn and accompanied by restlessness, add 15g of helianthus bark and 20g of nightshade vine. 2. Analysis of the formula This formula is derived from the Four Reversals Method [1] of Professor Lu Chonghan, a contemporary Fu Yang expert, plus the addition and subtraction of Fengming Dan and Gui Gan Longmu Tang. The bitterness of ginger is stronger, which can enhance the warming and subduing effect of this formula. The addition of Atractylodes macrocephala, together with roasted licorice, replenishes earth and fire, so that the Yang is sealed and does not dissipate. Wood Butterfly can strengthen the submerging effect of Marrow Sealing Dan and can be regarded as a potentiator of Marrow Sealing Dan. Dragon bones, oysters and dragon teeth calm the mind and subdue deficient yang. Besides, Dragon Bone and Oyster are in the sky as Dragon, and in the sea as Kai, both of them can be used together to transport Yin and Yang, which is a good match for treating insomnia. The combination of Gui Zhi with Gan Cao and Long Mu is the meaning of Gui Zhi Gan Cao, Dragon Bone and Oyster Soup, which can subdue the heart yang and treat deficiency annoyance, and Gui Zhi can go to the muscle surface and upper jiao, dispel the cold evil, and open the way for the return of Yuan Yang. Complemented by sour jujube to benefit the heart and nourish the liver and calm the mind, Zhu Fu Shen to calm the heart and mind, and traffic the heart and kidneys, the drugs work together to warm and submerge the floating yang, guide the dragon into the sea, traffic the heart and kidneys. This formula is clinically used for treating insomnia caused by “deficiency of yang energy, internal flourishing of yin-cold, ge-yang in the upper part of the body and non-interaction between the heart and kidney”. Symptoms include difficulty in falling asleep, or waking up easily after sleeping, dreaming a lot, with cold feet, no thirst in the mouth, or thirsty and preferring hot drinks, nocturnal and sleepy eyes, low voice and short breath, little breath and lazy speech, heavy body and fear of cold, pale and fat tongue with tooth marks, white greasy and lubricating coating, thin and weak pulse or floating and large and empty, accompanied by dry throat, crusty mouth, toothache and swelling, and other forms of deficiency fire on the head and five senses.
