Anxiety disorder and Chinese medicine “Penguibi” has two meanings in Chinese medical literature. The main clinical manifestation is “the pain does not leave the front and back” and “the pain does not leave the front and back, and the up and down has an end”, which is similar to the modern intestinal obstruction. For example, the “Spiritual Pivot – Evil Qi Viscera Disease Form” cloud kidney pulse “slightly urgent for the sinking convulsions running dolphin, the foot does not collect, not before and after.” Ma Maki note: “and for the dolphin, to kidney evil gradually accumulate and become, for the foot does not receive, to kidney qi travel in the foot also, for not before and after, to kidney orifice in the two stools also.” Su Wen – Bone and Space Theory: “This disease, from the less abdominal up the heart and pain, not before and after, for the punch hernia.” The “Difficult Classic – 56 Difficult” cloud: “The accumulation of the kidney, called ben dolphin, develops in the less abdominal, up to the heart, like a dolphin, or up or down without time, for a long time, causing wheezing and rebellion, impotence of the bones and less gas, obtained on the day of summer C Ding.” The “Difficult Classic・Fifty-five Difficult” says: “The accumulation, Yin Qi, also, its beginning has a regular place, its pain does not leave its part, up and down has the beginning and end, left and right has a poor place.” Secondly, it is caused by the seven emotions of fright and fear, as well as by sweating and burning needles, and is clinically characterized by “Qi rushing up to the chest and throat,” “seizures that want to die, but stop again,” “abdominal pain” The main clinical manifestations are “cold and fever”, “palpitations under the umbilicus and palpitations of the heart” and so on. For example, in the chapter of Jin Kui Yao – Penguiculosis, it is said that “Penguiculosis starts from the lesser abdomen, rushes upward to the throat, seizures want to die, and stops again, all from panic” “Penguiculosis rushes upward to the chest, abdominal pain, cold and fever, Penguiculosis is the mainstay of the soup”. After sweating, burn the needle to make it sweat, the needle place is cold, the nucleus up and red, must be sent running dolphin, gas from the less abdominal up to the heart, moxibustion on its nucleus each a strong, and Gui Zhi plus Gui Tang main.” Chao Yuanfang, “Treatise on the Origin of Diseases – Ben Dolphin Climate” states, “Ben Dolphin Qi is the accumulation of Qi in the kidneys. It starts from panic, worry and thought. If frightened, it hurts the spirit, the heart collects the spirit. If you are worried, it hurts the will, the kidney collects the will. The spirit is injured and moved, the gas accumulates in the kidney, and the gas travels downward, like the running of a dolphin, so it is called running dolphin. The gas multiplied by the heart, if the heart is enthusiastic as things are frightened, as people are afraid, five Tibetan uncertainty, food and drink often vomit, full of gas in the chest, madness and uncertainty, delusional speech and delusional vision, this panic Ben dolphin of the state. If the gas is full of branches of the heart, the heart under the boring chaos, do not want to hear the sound of people, rest sometimes, the first disease and the first extreme, short breath, hand and foot syncope, inner pain, warmth and desire to vomit, this worry pent-up dolphin of the symptoms.” Although Chao’s theory of pent-up dolphin includes the theory of kidney accumulation, the description of symptoms in the text shows that “wild and unstable” and “delusional speech and delusional vision” are typical manifestations of neurological diseases, which are very common in neuropsychiatric diseases. This has certain significance in guiding the diagnosis and treatment of this disease. Zhang Lu, a famous doctor of the Qing Dynasty, questioned the “kidney accumulation” of pangolin and said in Zhang’s Medical General: “Although pangolin is said to be kidney accumulation, it is actually a problem of the Chong pulse. …… set up mud Penguibi for kidney accumulation and use the agent of felling the kidney is wrong.” It is suggested that the disease is a functional lesion, which is also known as “the disease of Penguiculosis” by Zhongjing. The typical clinical manifestations of patients with acute anxiety disorder (panic attack) are: sudden onset of strong discomfort, such as chest tightness, breathless feeling, palpitations, sweating, stomach discomfort, trembling, numbness of hands and feet, sense of dying, sense of going crazy or loss of control, and in severe cases, running and shouting, with each attack lasting from a few minutes to tens of minutes and relieving on its own. It is very similar to the above-mentioned “Bengui Qi disease”. It is worth noting that some patients with epilepsy also present similar manifestations, such as visceral sensory seizures in complex partial seizures in epilepsy, which often manifest as a feeling of heat rising from the abdomen or chest toward the head, and palpitations, abdominal pain, bowel sounds, and a sense of urgency; emotional disorder seizures, which often manifest as fear, anxiety, restlessness, anger, depression, or euphoria. Clinical identification can be based on the presence or absence of loss of consciousness. Those with clear consciousness during seizures and able to recall after seizures are panic attacks; those with loss of consciousness during seizures and unable to recall after seizures are epilepsy. In addition, from several formulas created by Zhongjing for the treatment of prandial qi disease, all of them have sedative components, which are very suitable for the treatment of complex partial epilepsy and panic attacks of anxiety disorders. For example, 50 mg/(kg・d) of total glucoside of peony (TGP) from peony, the main drug in Gui Zhi Jia Gui Tang, can not only prolong the duration of slow-wave sleep in normal rats by gavage for 7 days, but also restore the sleep parameters of caffeine-induced insomnia rats to near normal levels, and significantly prolong the total duration of slow-wave sleep and heterophasic sleep in swimming rats. Cinnamaldehyde contained in cinnamon has a significant sedative effect on mice. Cinnamon can relieve panic, palpitation, chest tightness, dyspnea and sweating during anxiety attacks, and has a good effect on the treatment of anxiety disorders. Therefore, in addition to being associated with a small number of complex partial epilepsies, the Chinese medicine “Pengyu qi disease” is closely related to panic attacks in anxiety disorders.
