My CT diagnosis is lumbar disc herniation, so my back pain must be caused by “lumbar herniation”. Analysis: There are many causes of low back pain, and more and more people are being diagnosed with “lumbar disc herniation” in the radiology department’s CT diagnosis. In fact, the proportion of asymptomatic lumbar disc herniation in the normal population may be as high as 50% or more, the intervertebral disc begins to degenerate after the age of 20, which is a very normal physiological phenomenon, the real lumbar pain caused by “lumbar herniation” is actually not a lot in clinical practice. The root cause of most lumbar pain is neither in the “disc” nor in the “bone spur”, but in the strain on the soft tissues of the lumbar region. For cervical and low back pain, any conservative treatment is to treat the symptoms, only surgery is to treat the root cause, and surgical treatment can be done once and for all. Analysis: Admittedly, a very small number of patients with neck and low back pain who are ineffective by conservative treatment do need surgery. However, as far as the current medical level is concerned, chronic neck and low back pain is still one of the “three persistent diseases” that plague human beings, and it is impossible for any kind of treatment to solve the problem once and for all. Compared with conservative treatment, surgery is more traumatic to the human body and has a longer recovery period. At the same time, it also changes the inherent biomechanical balance of the human spine, making some spinal segments appear more fragile. Thus, except for some extremely serious neck and low back pain disorders where strict, regular conservative treatment for 3-6 months is ineffective, surgical treatment must be chosen with caution. Painkillers can only treat the symptoms and have side effects, thus I will not take medicine for localized pain again. Analysis: In many acute neck and back pains, the key lesion lies in the onset of acute aseptic inflammation. In the early stage of the attack, taking anti-inflammatory and pain-relieving drugs in time is very good for eliminating the aseptic inflammation, stopping its development and preventing it from becoming chronic pain. For patients, no matter what method is used, eliminating pain and improving quality of life is the fundamental purpose of treating neck and back pain. The plain film and CT of my cervical spine are fine, so there is no problem with my cervical spine. Analysis: The fact that the plain film and CT of the cervical spine are fine only means that the bony tissues of the cervical spine do not show problems on the film, but the lesions of the soft tissues around the cervical spine cannot be diagnosed by general plain film and CT, and certain early bony lesions are not visible in the film. Thus, any radiological diagnostic information can only be used as a reference for the doctor to diagnose the disease, but cannot replace the clinical diagnosis made by the doctor, and in clinical practice, inconsistency between clinical and radiological diagnosis actually occurs frequently.
