The stones are brownish-black in appearance, soft, friable, and vary in shape, size, and number. Some of them look like fine sand or unshaped mud, so they are called “mud-like stones”. These stones are pigmented stones mainly composed of calcium bilirubin. The main components are bilirubin, biliverdin and a small amount of cholesterol, as well as calcium, sodium, potassium, phosphorus, magnesium and other minerals and a variety of trace elements. Among the minerals, calcium ions have the highest content and are easily combined with bilirubin to form calcium bilirubin, in addition to a variety of proteins and mucins forming a reticular scaffold. Some of them are visible under the microscope, such as the shell skin of parasites, eggs and bacterial aggregates. Why is it an untimely bomb? 1, because the common bile duct stones can easily lead to obstruction of the common bile duct and pancreatic duct, causing acute obstructive purulent cholangitis or acute pancreatitis, which have rapid onset, rapid progression and high mortality; 2, when the common bile duct stones cause bile duct obstruction, there is no pattern, cannot be prevented, and may attack at any time; 3, if the patient is accompanied by other underlying diseases, such as diabetes, serious cardiovascular disease, etc., once the acute attack The condition is often difficult to control and endangers the patient’s life.
