CAG often evolves from chronic superficial gastritis. In the early stage, there are no obvious symptoms, and the symptoms of gastritis usually appear after the atrophy of the gland, which are often manifested clinically as distension, pain, belching, heartburn and indigestion in the upper abdomen. In Chinese medicine, CAG belongs to the category of “gangrene, gastric pain”, which is located in the stomach, closely related to the spleen and liver, and is a disease of deficiency and symptoms. Clinically, it is often manifested as a mixture of deficiency and actuality, with spleen and stomach weakness as the basis and qi stagnation and blood stasis as the symptoms. Since CAG has no specific clinical symptoms and signs, its diagnosis mainly relies on endoscopy and histological examination of gastric mucosa biopsy, especially the latter is of greater diagnostic value. There are two types of gastric mucosal atrophy histologically: (i) chemogenic atrophy: the intrinsic gastric glands are replaced by enteric or pseudopyloric glandular chemogenic glands; (ii) non-chemogenic atrophy: the intrinsic glands of the gastric mucosal layer are replaced by fibrous or fibromuscular tissue or the number of intrinsic glands is reduced by inflammatory cell infiltration. In terms of treatment, it is easier to improve the symptoms, but more difficult to achieve pathological improvement, especially for intestinal epithelial metaplasia and heterogeneous hyperplasia to make a breakthrough, such as tackling the difficulties. According to the characteristics of CAG, such as long duration of disease, mixed pathogenesis, and interchangeable evidence types, and according to the “Long-term disease into the luo” and “Long-term pain into the luo” proposed by Ye Tianshi in the “Clinical Guide to Medical Cases”, as well as the “disease after several years, has entered the stomach luo “It is believed that CAG is a disease of both blood and qi, with the blood being the main cause. The evil of cold and dampness is found on the surface of the muscles and does not go away, passing into the meridians and entering the gastric lobe; or the evil of cold and dampness is found in the internal organs and meridians; or the spleen and stomach are weak and the evil of cold and dampness stays in the stomach, the evil stays, the disease enters the lobe for a long time, the gastric lobe is stagnant, the lobe is deficient, and the stomach loses its nourishment and glory. The underlying mechanism of the disease is “weakness of the spleen and stomach, and the evil of cold and dampness, which deeply enters the stomach lobe. Accordingly, Master Zhou often adopts Jianshu Tongluo Tang, which has the function of benefiting Qi and strengthening the spleen, warming cold and dampness, and pungent and warming the ligaments, to treat it. This formula is based on Si Jun Zi Tang with the addition of flavors, consisting of Dang Shen, Bran Fried Atractylodes, Poria, Raw Astragalus, Sandy, Preparedness, Sorozi, Fa Han Xia, Gui Zhi, Dry Ginger, Gan Song, Dan Shen, Fried White Peony, and Honey Licorice. Party ginseng and Atractylodes macrocephala benefit the spleen; Poria can not only benefit the spleen and stomach, but also permeate water and dampness, and Astragalus in combination, can increase the function of strengthening the spleen and Qi; Fa Xianxia pungent and warm dry, can dry dampness and phlegm, and stomach and vomiting; Sha Ren pungent and warm, can transform dampness and stomach, warming the spleen; Gui Zhi pungent and warm, warming the meridian and pass through, help Yang Qi, dispersing cold and pain, and Atractylodes macrocephala, Poria, licorice used together, to warm the spleen Yang, transform dampness and water; Gan Song, dry ginger, pungent and warm They are also used with Atractylodes macrocephala, Poria cocos and Glycyrrhiza glabra to warm the Spleen Yang, resolve Dampness and relieve water; Gansong and Ginger, pungent and warm, can warm the middle and disperse cold; Salvia miltiorrhiza invigorates Blood stasis, unblocks the meridians; Paeonia lactiflora nourishes the Blood and the meridians, eases the pain. The whole formula is effective in benefiting Qi, strengthening the spleen, warming the middle and relieving pain, resolving dampness, harmonizing the stomach and activating blood circulation, with the characteristics of taking care of both the symptoms and the root cause. By tonifying the spleen and stomach, dispersing the cold and dispersing the blood stasis, the stagnation in the stomach and ligaments can be removed, the qi can be regulated, the evil can be removed and the righteousness can be restored, thus treating the weak spleen and stomach type CAG. It is generally believed that the reconstruction of the atrophic glands takes 3-5 months, so the treatment course for CAG is longer. Most physicians advocate 6 months as a course of treatment, with 1-3 courses of treatment observed depending on the severity of the disease. For severe atrophy or with severe intestinal epithelial hyperplasia or anisotropic hyperplasia, it is advisable to extend the course of treatment and adhere to regular review for a long time to determine the condition. This study showed that after 12 weeks (3 months) of treatment with Jianshu Tongluo Tang, patients with CAG with weak spleen and stomach showed significant improvement in gastric mucosal atrophy and activity, with a pathological healing rate of 43.33% and an overall effective rate of 83.33%, which shows that Jianshu Tongluo Tang has a good reversal effect on gastric mucosal atrophy in patients with CAG with weak spleen and stomach.
