Elderly people should guard against neuralgia caused by shingles

  In recent times, there has been a sudden increase in the number of patients with post-herpetic pain. I think it may be due to the weather, the high temperatures this summer, the chronic illnesses of the elderly themselves, and the lowered resistance in the difficult climate, which may increase the incidence of such diseases. The herpes zoster virus is normally present in the body and takes advantage of fatigue, rapid emotional changes and reduced resistance.  The reason it is called herpes zoster is because it often occurs in the lower back, like a loincloth, so it used to be called “waist-wrapping dragon”. Nowadays, the onset of herpes zoster has changed a lot and has advanced with the times, not only in the waist, chest and abdomen, but also in all parts of the body from the head to the feet, as long as there are nerves and skin, especially in the armpits, the head and face, the occipital area and the perineum, which are the most difficult to treat; many patients have pain before the herpes appears, mostly burning, electrical, hot pain, and even pain when the wind blows, etc. Herpes usually appears a week to two weeks after the onset of pain.  Herpes simplex itself is not terrible, the rash disappears after a few days, it is the pain that is scary. If the pain is seen during the pre-herpetic period, with treatment, the pain can be well contained and disappear as the herpes disappears. If not treated promptly it can persist for some time or even years after the herpes disappears and is called postherpetic neuralgia, which becomes more difficult to treat the longer it goes on. The longest history of the disease in our patients is more than ten years. Many patients with severe or prolonged pain are even suicidal, and the most common phrase is “I’d rather die”. The cause of pain is herpes virus invasion of nerves, resulting in demyelination of nerves. In the past, nerve damage was thought to be irreparable, but now research has found that damaged nerve tissue can be repaired under specific experimental conditions, but this process is extremely long and difficult to achieve in humans. This is why early treatment is especially important.  Treatment: Postherpetic neuralgia is not effective with general painkillers, and any single drug or method cannot achieve the desired therapeutic effect, even in the specialized field of pain medicine, which is a relatively tricky disease. The treatment emphasizes a multimodal, three-dimensional and comprehensive treatment. It is necessary to: 1. combine the application of drugs with multiple routes of action; 2. apply several different physical therapy and drugs according to the course of the disease; 3. perform selective peripheral receptor block according to the characteristics and location of the pain; 4. apply the “upper leadership” of the corresponding “competent” lesion site “The pain can be controlled in the shortest possible time by intervening, nourishing, blocking, regulating, and even destroying the nerve root or nerve trunk. Many of these tools are specific to pain medicine.  So, a reminder to our patients suffering from shingles pain: if you have a pain department in your neighborhood, be sure to ask for a pain department as soon as possible.
