Heat therapy is currently one of the most commonly used physical therapy methods for the clinical treatment of lumbar spondylosis (especially chronic lumbar spondylosis). Heat therapy is divided into superficial heating and deep heating, and it is most appropriate to control the temperature at 40℃~45℃ during treatment. Too high a temperature will cause tissue damage. Pain during heat therapy is a sign that the safety range has been exceeded. The mechanism of heat therapy for the treatment of low back pain: ① can change the physical properties of collagen tissue, increase the extensibility of tendons, ligaments and joint capsule, and reduce joint stiffness; ② make small arteries and capillaries dilate, increase the local blood flow, increase the permeability of capillaries, and facilitate the absorption of hematoma, edema and exudate; ③ improve the pain threshold of nerve endings (i.e. the sensitivity of nerve endings receptors to pain stimulation), thus achieve the purpose of pain relief; ④ reduce the sensitivity of muscles to pulling stimuli and eliminate muscle spasm, thus improving the pain and activity disorders caused by muscle spasm. Heat therapy is effective for lumbar pain caused by chronic lumbar muscle strain, chronic supraspinous or interspinous ligament strain, lumbar small joint disorder, acute lumbar sprain, lumbar disc herniation, lumbar spinal stenosis, sacroiliac joint causing acute osteitis, lumbar dorsal fascia fibrositis, ankylosing spondylitis, etc. A water ironing method with hot water bags filled with hot water, (generally water temperature at 50 ℃ ~ 60 ℃), drain the air, cover tight plug, wrapped in a towel, placed on the waist back or buttocks, moving back and forth, stay longer in the affected area. Each time 10 to 20 minutes, 3 to 5 times a day. Second, salt ironing method with 500 grams of coarse sea salt, put into a pot of acute fire dry frying for about 5 minutes, until the salt is yellow and hot, pour out with white paper wrapped in a cotton cloth. Third, wax therapy Prepare a 500 ml infusion soft packing empty bag, 450 grams of white wax, a 50 ml syringe, a large needle with 15 gauge for injection, an enamel cup and an alcohol lamp. First, put the white wax into the enamel cup and heat it with a gentle fire until the wax melts completely into liquid state. Insert the 15 gauge needle into the infusion bag drip tube, then use the syringe to draw in the wax, and then inject 350-400 grams into the infusion bag through the large needle. Pumping wax and syringe wax action should be fast to prevent the wax liquid from solidifying and not being injected. After each syringe injection, a small amount of air should be pumped into the soft bag drip tube to prevent the remaining wax in the needle and drip tube from re-consolidating due to lower temperature. After wax injection, empty the bag of air, and the infusion bag drip tube knotted spare. Place the prepared wax bag in hot water until the wax is completely melted into a liquid state and then remove. Wipe the wax bag dry, put it into a cotton bag, and put the wax bag on the affected area of the lower back for 1 hour each time, 2 to 3 times. Fourth, Kanli sand therapy Kanli sand is prepared with vinegar and iron sand, plus Chinese herbs such as Fangfeng, Angelica, Chuanxiong and Turbinate, which are available in hospital medicine or pharmacies. Put a bag of modulated and heated kanli sand bag on the corresponding affected area. Apply hot compresses 1 to 2 times a day for 20 to 30 minutes each time. Fifth, warm water bath therapy Warm water bath therapy used to control the water temperature at about 40 ℃, the patient’s whole body immersed in it, first soak for 5 minutes, self-massage waist. Generally soak for about 15 minutes each time the hand, once a day. Can also keep the room temperature of about 40 ℃, while showering with a home water heater, the water temperature to make the local skin flushed and comfortable not hot as degree. Patients who have the conditions can choose hot spring bath therapy, the effect is better. Sixth, medicine bath therapy Medicine bath therapy is one of the unique therapies of traditional Chinese medicine in the treatment of low back pain, which is based on the warming effect of warm water bath with different Chinese herbs, so that the drug can penetrate through the skin to the affected area to improve blood circulation and accelerate the role of metabolism. Medicinal bath should be tailored to the person, the following are several medicinal bath methods: 1, ginger 50 ~ 100 grams, cut into thin slices, into 500 ~ 1000 ml of hot water (about 60 ℃) for 10 minutes, to be soaked in ginger juice, to clean gauze dipped in ginger water in the chest and back, waist repeatedly scrubbed. Each time 15-30 minutes, 1 to 2 times a day. Where high fever, purulent inflammation, cardiac and renal insufficiency, skin allergies, dermatitis, tumors, tuberculosis and other patients, heat therapy is not recommended.
