Traditional Chinese medicine is the treasure of traditional Chinese cultural heritage, and osteoarthritis, as a part of orthopedics, has accumulated a lot of experience in treatment. I have done a lot of analysis of ancient literature on osteoarthritis in my graduate studies and summarized the ancient understanding of osteoarthritis, which I share below. Ancient physicians classified the swollen joints, pain, numbness, wood, heaviness, stiffness, deformity, and muscle atrophy caused by osteoarthritis under the category of “paralysis,” and the names of diseases related to osteoarthritis include “paralysis,” “bone paralysis,” and “bone paralysis. Bone paralysis”, “knee paralysis”, “joint paralysis”, “crane knee wind”, “impotence” or “impotence”. impotence” or “impotence paralysis”, etc. Chinese medicine treats osteoarthritis by emphasizing the local and the overall, mainly by mobilizing the internal factors of the body to fight against the paralysis. The treatment is based on the principle of “the root cause of the disease must be sought”. In terms of the cause and symptoms, the cause is the “root” and the symptoms are the “symptoms”, so the treatment should take into account both the root and the symptoms to help eliminate the evil. The treatment of paralysis in Chinese medicine is both holistic and local; the second is to take into account both the symptoms and the root cause. The internal treatment method is mainly based on the main method of removing the evil, opening the ligaments and helping the positive. The main treatment is to dispel the wind, which is the main cause of paralysis. The treatment is represented by Feng Feng Tang. The Shengji General Record, Volume 2, records “Fangfeng Tang”. Composition: Fangfeng (remove fork), licorice (sizzling, file, one tael each), scutellaria (remove black heart, three points), angelica (cut, roasted), red poria (remove black skin, one tael each), gentiana (remove seedling, earth), gentiana (file, three points each), cinnamon (remove rough skin), almond (remove skin tip, double kernel, fried, one tael each), ephedra (remove root knot, decoction, remove foam, roasted, half tael). Usage: The above eleven flavors, coarse pounding sieve, each serving five money dagger, wine, water, three broken dates, five slices of ginger, the same decoction to a jan, remove the dregs, warm, day two night one. Effects: Dredging wind and activating collaterals, promoting paralysis and relieving pain. Indications: walking paralysis, walking unsteadiness. 2) Dispersing cold Dispersing cold is mainly for the treatment of “painful paralysis” which is mainly caused by cold and dampness. It is represented by Wu Tou Tang. On the upper part of the volume of “The Essentials of the Golden Killing” is “Wu Tou Tang”. Ingredients: 9 grams each of ephedra, peony, astragalus, licorice (hot), 6 grams of Chuan Wu (hey chew, 400 ml of honey, decoction of 200 ml, that is, out of the head of Wu). Usage: The above five flavors, hey chew four flavors. Take 600 ml of water, boil 200 ml, remove the dregs, nano honey decoction, more decoction, serve 140 ml, do not know, take all. Indications: Pain in the feet, unable to flex and extend. Cold and damp paralysis. Treatment of symptoms: severe pain in the joints, inability to flex and extend, fear of cold and preference for heat, thin white tongue coating and sunken string pulse. 3) Removal of dampness Removal of dampness is mainly aimed at the treatment of “paralysis with dampness”, which is mainly caused by the sensation of wind, cold and dampness. It is represented by Job’s Tears. The source of Job’s Tears is “Puji’s recipe” Volume 3. Composition: Job’s tear (one tael), angelica (washed, remove reed, thinly cut, roasted), small Chuanxiong, ginger (cannon), licorice (sizzling), Guan Gui (remove rough skin, not see fire), Chuan Wu (cannon, remove skin tip), Fang Feng (remove hairpin strands), Yin Taro (remove stems, file, fry), ginseng (remove reed) Qiang Wu (remove reed), white atractylodes, ephedra (remove root), Dou Wu (yellow like ghost eyes, washed, remove reed, roasted, half a tael each). Usage: Finely powdered, two coins per serving, empty heart, lying down, mixed with wine, three times a day. Effects: Dispelling wind and dampness, warming the kidney and nourishing the blood, relieving tendons and pain. Indications: Insufficient liver and kidney, wind and dampness flowing into the tendons and bones, soreness and pain around the body, unfavorable limbs, or swelling and pain in the fingers and toes, unfavorable flexion and extension. 4) Clearing heat Clearing heat is mainly aimed at the treatment of “heat paralysis” which is caused by feeling wind, cold and dampness, and which is prolonged and turns into heat. It is represented by Bai Hu plus Gui Zhi Tang. Bai Hu plus Gui Zhi Tang is derived from “The Essential Guide to the Golden Horoscope”. Ingredients: Zhi Mu (six taels), Gan Cao (two taels, sizzling), Gypsum (one catty), Japonica rice (two heaps), Gui Zhi (peeled, two taels). Usage: File, each five money, one and a half calanders of water, decoction to eight minutes, remove the dregs, take warm. Effects: Clearing heat and relieving muscles. Indications: Warm malaria, its pulse is flat, the body does not have cold but heat, bone pain and annoyance, sometimes vomiting. Tongluo 1) Invigorate blood circulation Source from “Body Pain and Removal of Blood Stasis Soup” in the second volume of “Medical Lin Correction”. Ingredients: Gentiana macrophylla (1 yuan), Chuanxiong (2 yuan), Tao Ren (3 yuan), Safflower (3 yuan), Glycyrrhiza glabra (2 yuan), Qiang Wu (1 yuan), Myrrh (2 yuan), Angelica sinensis (3 yuan), Ling Fei (2 yuan fried), Xiang Shen (1 yuan), Niu Knee (3 yuan), Di Long (2 yuan removed from earth). Effects: Promoting blood circulation and eliminating blood stasis, promoting paralysis and relieving pain. Indications: Shoulder pain, arm pain, lumbago, leg pain, or circumferential pain caused by paralysis of qi and blood, which does not heal after a long period of time. 2) Cleansing phlegm The paralysis is prolonged, resulting in cold condensation and phlegm stasis, so it is often necessary to add phlegm-cleansing and stasis-removing drugs in the late stage of paralysis, such as Nanxing, Panax notoginseng, Tao Ren, Safflower, Andrographis paniculata, Turtle worms and Di Long. The spleen and stomach should be nourished and strengthened. The paralysis is prolonged, the external evil is long-lived, the vital energy is depleted, the qi and blood are lost, and the internal organs are dysfunctional. The treatment is to nourish the spleen and stomach, benefit the qi and generate blood. Commonly used formulas include Ginseng Ling Bai Zhu San and Gui Shen Tang. 2) Nourishing the liver and kidney Inadequate treatment or prolonged paralysis is often accompanied by deficiency of the liver and kidney in the later stages. For example, “Niu Knee Pill” from “Su Wen Ji Qi Yi Bao Sheng Jie”, Volume 2. Composition: Ox knee (in wine), Dioscorea Z, Eucommia (fried and shredded), Cistanches (in wine), Fructus Anorthium, Semen Cuscutae (in wine), Tribulus terrestris (equal parts each), Cinnamomum (reduced by half). How to use: Finely powdered, boiled in wine and pounded pork loin pills, Tongzi large, empty heart wine, five to seventy pills. Functions: nourish the liver and kidney, strengthen the muscles and bones. Indications: Kidney and liver deficiency, bone impotence can not get up in bed, tendons slow to hold. In ancient times, the internal treatment of paralysis was applied in a very flexible way. It is not possible to stick to a single cause. In particular, the late stage of paralysis is often characterized by a mixture of deficiency and deficiency, with the deficiency of the liver and kidney and the deficiency of both qi and blood. Therefore, comprehensive consideration is needed in the treatment. The representative formula is Duluxiaosheng Tang from “Qianjin Fang”. It is used to tonify the kidney and disperse cold, remove phlegm and stasis, and search for wind and meridians.
