I. Initial consultation
One day in May, a middle-aged male patient came to the clinic, well-dressed, but fat and with a lack of energy. He sat down lazily and said that he was quite tired recently and always felt unmotivated, fatigued and sleepy. It turned out that the patient had been socializing and drinking a lot recently. Upon inquiry, the patient was told that he had no previous history of specific diseases. He was given a blood screening test, and the results indicated that his blood lipids: triglycerides 20.67 mmol/L and total cholesterol 7.64 mmol/L, were significantly elevated. Combined with the patient’s symptoms, the diagnosis of mixed hyperlipidemia was finally confirmed.
II. Treatment history
Since the patient’s lipids were found to be significantly elevated through the examination, if not controlled immediately, it may cause dysfunction and diseases of several systems such as digestive system, cardiovascular system, and endocrine system. After communicating with the patient about the possible serious risks, he was recommended to be admitted to the hospital for evaluation and treatment, and the patient agreed to be admitted. After admission, the patient was given a complete uric acid, blood glucose, routine blood tests, liver function, kidney function, cardiac enzymes and other tests, all of which were indicated to be normal, and no abnormalities were found in the bilateral carotid ultrasound.
After ruling out contraindications to lipid-lowering drugs, I chose a lipid-lowering regimen for the patient. I told the patient that since his total cholesterol and triglycerides were significantly elevated, he needed to take oral fenofibrate capsules in combination with Rosuvastatin calcium tablets to lower both triglycerides and cholesterol. However, each drug needs to be started in small doses and taken separately in the morning and evening to avoid significant increases in blood levels and possible liver damage associated with the combination. During the treatment period, liver function and lipid changes should be reviewed regularly and the dosage should be adjusted in time. At the same time, dietary adjustment should be made to avoid high-fat food and cooking oil, switch to vegetable oil for frying and increase the intake of vegetables.
III. Treatment effect
After the patient was admitted to the hospital, he was given oral lipid-lowering drugs and dietary modification. Through a two-pronged approach, after 1 week of hospitalization, the patient felt significantly better in terms of fatigue and weakness, and his body was much lighter. The patient was discharged and instructed to continue regular outpatient review.
IV. Notes
After 1 week of recuperation, the patient’s overall condition improved a lot, and I, as the doctor in charge, was very happy to see his face gradually recovering.
However, in order to prevent the patient’s condition from recurring, I need to remind the patient that his improvement depends on strict dietary control and medication during this period. In addition, during the recuperation period at home, you should also pay attention to appropriate exercise, such as running or brisk walking, about one hour after meals every day to speed up food metabolism and also help improve blood lipids.
V. Personal insight
Although there is no longer a lack of food, the burden of excessive high-fat food on the body is more than what the body can compensate for. Usually early mixed hyperlipidemia has no obvious symptoms and is easily overlooked, but if not detected and treated in time, excessive lipids may cause acute pancreatitis, and the long-term accumulation of lipids in the walls of blood vessels is a hidden risk for the occurrence of cardiovascular and cerebrovascular diseases. However, in terms of treatment, since the significantly elevated lipids are closely related to the patient’s usual diet, it is necessary to combine drug control with a change in high-calorie diet in order to fundamentally reduce the body’s lipid metabolism burden.
