Head shaking during concentration may be due to the patient having idiopathic tremor, a disorder with no specific manifestations other than tremor. Most of the causes are unknown, some patients are genetically related, and 1/3 of patients have a family history and their immediate family members may have similar symptoms. In addition to head shaking, hand tremors can also be present, which are usually more pronounced when fixing a certain position or performing fine movements. In this case, it is recommended that the patient should have a cranial MRI, EEG, and rheumatologic and immunologic tests to rule out organic diseases. If the symptoms worsen progressively with age, propranolol can be considered to relieve the symptoms, but it is necessary to monitor the heart rate and keep the heart rate above 60 beats per minute.
