Clinical use of beam lasers

  I. What is the image beam laser
  It changes the traditional laser emission mode and adopts multi-point focusing laser technology, each pulse laser consists of nearly one hundred
  Each laser pulse consists of nearly 100 micro laser pulses, and each micro laser spot area is only 75-100um, which is a new laser treatment mode.
  Second, the characteristics of the beam laser.
  1, each time the beam laser acts on the skin, only a tiny area of the thermal shock, retaining part of the skin is not affected;
  2. The skin tissue that the beam laser acts on can remove pigmentation and stimulate collagen growth at the same time;
  3. The skin tissue that is not affected by the laser can be used as a thermal diffusion area to avoid possible side effects such as thermal damage.
  It can also promote the healing of the skin.
  3. Selection of treatment indications
  ①Acne scars, trauma scars;
  ②Big pores;
  ③Photodamaged skin or gray skin;
  ④Fine wrinkles around the eyes;
  ⑤ skin texture and elasticity enhancement;
  ⑥Chloasma;
  ⑦Pigmentation caused by trauma and other factors, etc.
  Note: To achieve a more satisfactory treatment effect, chloasma must be treated comprehensively, i.e. internal adjustment + laser + external medicine or products, and cannot rely on laser equipment alone. (Recommended medication or products: internal regulation: spot removal and menstruation regulation capsule, tosamax, etc.; external application: arbutin, levulinic acid, hydroquinone cream, etc.)
  IV. Preparation before treatment.
  1.Preparation of patients: Anyone who belongs to one of the following conditions cannot receive the treatment of image beam laser (Pixel2940nm)
  ①Patients with skin cancer;
  ②Keloid patients;
  ③Patients with coagulation disorders or taking anticoagulants;
  ④Patients with acute skin diseases or taking photosensitive drugs;
  ⑤Patients with exemption defects;
  ⑥Patients with psychological disorders;
  (7) Pregnant women;
  ⑧Patients who have recently received sun exposure or are about to receive sun exposure;
  ⑨Patients with abnormal developmental nevi or malignant nevi in the treatment area;
  ⑩For patients who have used various chemicals or cosmetics containing mercury and lead to treat melasma before, it is recommended that treatment be carried out after three months (patients with moderate to severe melasma can use hydroquinone preparations on the spotted area one week before surgery);
  2.Preparation of doctors: Carefully check the lesioned parts of patients and judge whether they are suitable for Pixel2940nm treatment, i.e. the grasp of indications, and at the same time for melasma patients and pigmented patients must make clear the effect of treatment and individual differences, so as to avoid excessive expectations of patients.
  3. Preparation of skin condition: The better the hydration condition of the skin during treatment, the more obvious the improvement will be after treatment. Therefore, before the treatment can be appropriate according to the state of the patient’s skin using active spring water wet compress (Avène, Skin Spring are available) or moisturizing mask (seaweed, beauty, collagen mask) 15-20 minutes to correct the dry state of the skin.
  4.Preparation of surface anesthetic drugs: For patients with poor pain tolerance or more treatment sessions, lidocaine gel can be applied for 60–90 minutes before treatment to relieve the pain during treatment.
  5.Preparation of treatment mode.
  The Pixel 2940nm beam laser is available in two different treatment modes: 7X7 (49 microspots) and 9X9 (81 microspots.) The 7X7 treatment head produces 28 mJ/microspot of energy and the 9X9 treatment head produces 17 mJ/microspot of energy. Between 50 and 150 microns, the smaller the pixel volume (49 pixels vs. 81 pixels) the greater the depth of penetration.
  6.Pulse and frequency settings
  ①Pulse.
  Short – short pulse setting (default), energy is 100 to 800 mJ/P adjustable, growth unit is 100 mJ/P.
  Medium – medium pulse setting, energy is 100 to 1000 mJ/P adjustable, growth unit is 100 mJ/P.
  Long – long pulse setting, energy is 100 to 1400 mJ/P adjustable in medium, growth unit is 100 mJ/P.
  ②Frequency: 2 Hz
  7.Treatment techniques.
  ① Fixed point type (fixed point emission): Pixel 2940nm treatment head is fixedly placed on the treatment site and
  to be continuously emitted.
  ②Sliding type (mobile emission): the Pixel 2940nm treatment head is freely moving in the treatment area, and it should be continuously
  The treatment head of Pixel 2940nm is free to move around the treatment site, and it should be fired continuously.
  V. Treatment points.
  1.The skin of the treatment area must be thoroughly cleaned and kept dry during treatment.
  2. According to the specific clinical indications, set the treatment parameters, including the settings of light spot mode, energy, pulse width, treatment technique and number of treatment sessions.
  ①Acne scars, trauma scars: spot mode: 7X7 or 9X9, 7X7 is the main, energy 800-1200mj/p, pulse width: medium pulse width or long pulse width, treatment method: 6-8 pulses are emitted continuously in the treatment area.
  ②Pore enlargement: spot mode: 7X7 or 9X9, energy 800-1000mj/p, pulse width: medium pulse width, treatment technique: 3-6 pulses continuously emitted in the treatment area or sliding 3-4 times in the treatment area.
  ③Light aging: light spot mode: 7X7 or 9X9 or 7X7+9X9, energy 800-1000mj/p, pulse width: medium pulse width, treatment technique: sliding 2-3 times in the treatment area.
  ④Fine wrinkles at the corners of the eyes: spot mode: 7X7 or 9X9, with 7X7 as the main mode, energy 800-1000mj/p, pulse width: medium pulse width, treatment technique: 3-6 pulses are continuously emitted at fixed points in the treatment area.
  ⑤ Skin texture and skin tone enhancement: 7X7 or 9X9 or 7X7+9X9, energy 800-1000mj/p, pulse width: medium pulse width, treatment technique: 2-3 pulses are continuously emitted at fixed points in the treatment area or sliding 2-3 times.
  (6) Melasma: Light spot mode: 9X9, energy 600–800mj/p, pulse width: short pulse width or medium pulse width, medium pulse width mainly, treatment technique: sliding 2-3 times in the treatment area.
  (7) pigmentation: spot mode: 9X9, energy 800–1000mj/p, pulse width: medium pulse width, treatment technique: 3-4 slides in the treatment area. 3. The TOP head of Pixel’s handpiece must touch the skin vertically during the treatment, no forceful pressure, and no suspension. If it is a sliding technique, 50% overlap is allowed. If it is a multi-times sliding treatment, the direction of sliding can be flexibly adjusted, i.e. one time is horizontal and one time is vertical.
  4.During the treatment process, the factors that affect the skin reaction: the water content of the skin, the treatment spot pattern, the treatment technique, the number of times of treatment, the thinness of the skin stratum corneum and individual differences. If the reaction is obvious, the effect is obvious.
  Sixth, the skin reaction after treatment
  Immediately after treatment, small white spots will appear on the treatment site; 1-2 minutes patients will feel a burning sensation; 10-20 minutes redness (erythema) will appear on the treatment site; 2-3 days a thin layer of scabs will form on the treatment site; 5-10 days the scabs will fall off naturally (generally about 7 days).
  Immediately after treatment 5 hours after treatment
  Seven, the immediate treatment of skin care: can minimize redness, soothe pain, reduce skin temperature, to avoid the occurrence of hyperpigmentation.
  1.Wet compress of living spring water (0-4℃) (Avène, Skin Spring are available) or Traner collagen dressing paste (refrigerated) or apply epidermal growth factor gel (redness and swelling are not obvious after treatment); Note: The use of Traner collagen dressing paste immediately after treatment has a significant effect of relieving pain.
  2.Heavier pain, external application of gauze soaked with 2% lidocaine can relieve the immediate post-treatment pain
  3.For heavy skin redness and swelling after treatment, you can apply soft hormone ointment (such as Ketin Skin or Yuzor) and apply cold compresses at the end of treatment, and topical hormone ointment can be used three times a day for three days, or you can also take oral non-steroidal anti-inflammatory agents (such as Cilazol).
  VIII. Treatment course.
  1 time in 3-4 weeks, 4-6 times/course.
  IX. Post-treatment precautions.
  1, the day of treatment to avoid washing, from the next day onwards can be normal cleaning, pay attention to the cleaning action should be gentle, do not force to rub
  2, 2-3 days after the treatment site will appear very thin scab, generally 7 days will be completely off; scab off
  When the scab falls off, let it fall off naturally, do not use your hands to pick the scab to avoid leaving scars and pigmentation;
  At the same time, we should fully communicate with the patient, do not mistake the scabbing period for pigmentation or color aggravation;
  3. Do not swim, sauna or exercise excessively within 1 week after treatment;
  4, 1 week after treatment can be a daily wet compress or hydrating mask, pay attention to the wet compress or remove the film after not
  Do not rub the scab with your hands to avoid premature scab loss.
  5.During the recovery period, the treatment area should avoid contact with sunlight, pay attention to sun protection, use sunscreen when going out (SPF 30 in summer, SPF 30 in other seasons).

  6, according to the different treatment methods, treatment times, as well as individual differences, after the scabs fall off some individual patients will appear color difference phenomenon, generally 2 – 3 weeks to return to normal, there are also individual patients after treatment will appear temporary pigment loss or pigmentation, generally 3-6 months will gradually return to normal, also available L-vitamin C import or external application.
