However, there are two major problems facing pediatrics in TCM: the difficulty of identifying evidence and the difficulty of taking medication. The author is engaged in the work of Chinese medicine clinics, clinical pediatric patients, through years of clinical practice, for pediatrics, these two major problems, summed up their experience as follows: a. The identification of common diseases in pediatrics pediatrics in ancient times, also known as mute, is that children, especially young children under 1 year old can not speak, even 3, 4-year-old children, their discomfort can not be very clearly expressed, so for the identification of a major Therefore, the clinical practice is particularly focused on diagnosis and pulse diagnosis. However, pediatric diseases are relatively simple, mainly respiratory system diseases and digestive system diseases, and as long as we master these common diseases, we can easily deal with 80% of children in the clinic. (A) respiratory system diseases one of the common diseases in pediatrics children that is, respiratory system diseases, such as fever, cold, cough, asthma, etc., common in Western medicine, upper and lower respiratory tract infections, lung infections, tuberculosis, pharyngitis, laryngitis, tonsillitis, etc., is summarized as follows 1, the common prescription of respiratory system diseases: children’s performance: fever, chills, clear runny nose, night sweats, kicking being, thin white coating, pulse floating tight number. Identification points: night sweating, kicking. Analysis of the evidence: vicious chill, fever and clear runny nose are Sun; night sweating and kicking are Yangming. It is identified as the evidence of Tai Qing Long Tang of Sun and Yang Ming combined disease. According to Article 38 of the Treatise on Typhoid Fever, if a person suffers from sun stroke with floating and tight pulse, fever, malignant chills, body pain, and irritability without sweating, Da Qing Long Tang is the mainstay. If the pulse is weak, sweating, evil wind, do not take it, take it, then the syncope, tendons, tigers and flesh p, this is also the reverse. Hu Lao said that Da Qing Long Tang is a heavy agent for sweating and removing heat, and should not be used for non-surface real heat and heavy evidence of evil. In the clinic, even if the child has fever, chills and sweating, as long as there is a floating tight pulse, night sweats or kicking, it can be used with confidence. The floating and tight pulse indicates that the blood and fluids are not deficient, so the sweat can be developed. However, the method of taking the medicine must be correct, “a serving of sweat, stop after serving.” Remedy: 18 grams of ephedra, 12 grams of cinnamon stick, 10 grams of almonds, 6 grams of roasted licorice, 45 grams of gypsum, 15 grams of ginger, 10 grams of jujube. Decoction method: cool water soak the medicine for 1 hour, open the pot on high heat and then change to low heat for 15 minutes, pour out the liquid, add cool water over the medicine a cross finger, decoction again, two decoction mixture, divided into 4 parts, every two hours to serve 1 part, half an hour after taking the medicine, ask the child to wear warm or reheat the compress, sweating slightly that is to reduce clothing, go to the hot compress, stop after serving, diagnose the pulse if slow, quiet, for the fever subsides, this is the so-called “If the pulse is quiet, it is not transmitted.” If after sweating, the pulse is still floating tight number, still fever, can take 1 more, generally no sustained hyperthermia, if the second time after a slight sweat and then fever, we must “look at its pulse, know what is wrong, with the symptoms of treatment.” 2.Da Qing Long Tang minus Ephedra evidence: the child’s performance: runny nose, nasal congestion, sneezing, night sweats, kicking the blanket. Key points of identification: night sweating and kicking, but no fever. Analysis of the evidence: nasal congestion, runny nose, sneezing is Sun; night sweating and kicking of the blanket is Yangming; identified as a combination of Sun and Yangming disease of Da Qing Long Tang minus Ephedra Tang. Herbs: 10 grams of ephedra, 10 grams of cinnamon stick, 10 grams of almond, 6 grams of roasted licorice, 15 grams of ginger, 10 grams of jujube, 45 grams of raw gypsum. Decoction method: Decoction method is the same as before. Dosage: 1 dose of medicine is divided into 4 equal parts and finished in 2 days. Because the child has no fever, the dosage of ephedra is 10 grams, which can also be used for those who see the above evidence after the fever has subsided. 3, Chai Hu Gui Zhi Tang evidence of the child’s performance: nasal congestion, runny nose, pharyngeal congestion, sweating, fever, chills, white fur, slow pulse floating (the reason why we say pharyngeal congestion, but not pharyngeal pain, because the children’s ability to express the disease is not strong, pain and pain is not expressed, so we try to focus on the patient’s objective performance, if the pharynx is congested, the patient breast milk children, only eat without swallowing, a pharynx that cry, you can think of pharyngeal pain). Key points of identification: congestion of the pharynx, nasal congestion, runny nose, sweating, and evil wind. Identification analysis: nasal congestion, runny nose, sweating, evil cold, fever for solar surface deficiency; pharyngeal congestion for Shao Yang, identified as a combination of solar and Shao Yang disease, Chai Hu Gui Zhi Tang evidence. Article 146 of the “Treatise on Typhoid Fever”: For six or seven days of typhoid fever, fever, slight malignant chill, annoying pain in the branch joints, slight vomiting, branching knots under the heart, and external evidence has not gone, Chai Hu Gui Zhi Tang is the mainstay. Article 263 of the Treatise on Typhoid Fever: Shaoyang is a disease of bitterness in the mouth, dryness in the throat and dizziness in the eyes. The sore throat here is the very dry throat. Radix Codonopsis pilosulae 24 g (24 g for fever, 12 g for no fever), Radix Panax notoginseng 6 g, Radix Codonopsis pilosulae 10 g, Radix roasted licorice 6 g, Radix Scutellariae 10 g, Radix ginger 15 g, Ziziphi jujubae 10 g, Radix cinnamomi 10 g, Radix paeoniae 10 g. Add 10 grams of orris if the pharynx is congested and 45 grams of raw gypsum if the lips are dry. Decoction method: the same as Da Qing Long minus Ma Huang Tang. 4.Han Xia Hou Pao Tang (Chinese Medicine) The child’s symptoms: cough, white and greasy coating, floating and slippery pulse. Key points of identification: cough and floating pulse. Analysis: Floating and tight pulse for Sun, cough with greasy moss and slippery pulse for Taiyin. Article 5 of Jin Kui Yao – Miscellaneous Diseases of Women: If a woman has sizzling slices in her throat, Han Xia Hou Pu Tang should be used. Hu Lao said: “In later times, the addition and reduction of this formula was called Si Qi Tang. It is only used to treat women’s emotions and depression or can be praised, but the lack of knowledge of ginger and perilla leaves to relieve the symptoms often cannot be used in the treatment of coughs and other coughs of external evil and internal drinks, which should be noted, such as using perilla seeds instead of perilla leaves to treat cold coughs better.” Remedy: 10 grams of raw semen, 10 grams of thick park, 10 grams of perilla seed, 10 grams of poria, 15 grams of ginger and 10 grams of loosestrife. Decoction: Same as Da Qing Long minus Ma Huang Tang. This formula is seldom used alone, but I use it in combination with Da Qing Long minus Ma Huang Tang and Chai Hu Gui Zhi Tang according to the child’s clinical performance. Because children mostly do not cough up phlegm, many children with cough and phlegm add dried ginger, hsiangxin and wu wei zi; if coughing is frequent and phlegm is not easy to cough up, add Chen Pi 30 grams. 5.Evidence of Xiao Qing Long Tang The child’s symptoms: cough, sputum, phlegm, wheezing, runny nose, white greasy coating and slippery pulse. Key points of identification: cough, wheezing and phlegm. Analysis of the evidence: runny nose for Sun; cough, phlegm, wheezing, white greasy moss and slippery pulse for Taiyin. Article 40 of the “Treatise on Typhoid Fever”: If the table of typhoid fever is not relieved, and there is watery qi in the heart, dry vomiting, fever and cough, or thirst or diarrhea, or choking, or unhealthy urination, or fullness of the abdomen or asthma, Xiao Qing Long Tang is used. Herbs: Gui Zhi 10 grams, Bai Shao 10 grams, Ephedra 10 grams, Gan Jiang 10 grams, Hosanna 6 grams, Sheng Han Xia 6 grams, Roasted Licorice 6 grams, Wu Wei Zi 10 grams. Decoction: Same as Da Qing Long minus Ma Huang Tang. Add 45 grams of raw gypsum for dry lips. 6. Evidence of ejaculation and ephedra soup The child’s manifestations: cough, phlegm sound (water chicken sound), wheezing, runny nose, congestion of the pharynx, dry lips, white greasy moss or slightly yellow greasy moss, weak and slippery pulse Key points of identification: cough, phlegm sound (water chicken sound). Analysis: cough and runny nose for Sun; phlegm sound (water chicken sound), wheezing, and white greasy moss for Taiyin; congested pharynx, dry lips, and yellowish greasy moss for Yangming. It is identified as a combination of Sun, Taiyin, and Yangming with the evidence of shooting dryness and ephedra soup. Article 6 of “Jin Kui Yao – Lung Impotence, Lung Pain, Coughing and Upper Qi”: Coughing with upper Qi and watery chicken sound in the throat is referred to as “Sugan Ma Huang Tang”. Herbs: 10g of Radix et Rhizoma eucommiae, 10g of Ephedra, 15g of ginger, 10g of jujube, 6g of pseudostellariae, 10g of dong Hua, 10g of Zingiber officinale, 6g of Semen parviflora, 10g of Schisandrae. Decoction: Same as Da Qing Long minus Ma Huang Tang.
