Post-arthroscopic meniscectomy or revision rehabilitation program

  1. The ankle pump exercise can be started on the day after the recovery from anesthesia after surgery by the following method: With both lower limbs straightened in bed, the patient should first relax both ankles naturally and then do dorsiflexion, and the dorsiflexion must reach its maximum. Then do plantarflexion from the maximum dorsiflexion state, and plantarflexion must also reach the maximum. Do this repeatedly. You can keep practicing plantarflexion and dorsiflexion while the patient is in bed watching TV, reading books or newspapers and talking with family members. 2. In addition to starting ankle pump exercises for the ankle joint within 24 hours after surgery, active exercises for the hip joint can also be started (30 times – 1 day). When performing ankle pump exercises and hip exercises, the knee joint remains stationary; however, quadriceps contraction and relaxation exercises for the operated knee joint can be performed (100 times per group, 3 groups/day). How to perform quadriceps contraction and relaxation exercises while lying in bed: The patient lies in bed with legs naturally straight, and repeatedly performs the activity of contracting the thigh muscles of both lower limbs for 5 seconds and then relaxing them for 2 seconds. The contraction and relaxation of quadriceps can be performed on both knees at the same time, which can increase the efficiency of quadriceps exercises by 30%. 2, the day after surgery, you can walk with crutches on the ground with full weight (daily walking time with crutches is 30 minutes in total, each walking time is about 5 minutes), quadriceps training from “static contraction training” to “straight leg raising training”. (20 times → 10 times → 5 times for 1 set, 1-2 sets/day, can be divided into morning and afternoon exercises). The knee joint can be micro-movement in the cotton pad from 0-30°. Other exercises are the same as the 24 hours after surgery. 3. On the third postoperative day, the knee joint can be moved at 30-60°. Other activities are the same as the second postoperative day. 4. On the third or fourth postoperative day, the wound should be changed from “cotton leg” to “elastic bandage” and you can start to walk on the ground without crutches, with the total walking time limited to 30 minutes per day and within 10 minutes per walk. That is, if you walk for 10 minutes each time, you can walk up to 3 times a day; if you walk for 5 minutes each time, you can walk 6 times a day. At the same time, the quadriceps contraction and relaxation training is performed. Flexion of the knee should be over 90 degrees, at which time the patient can be discharged. 5. 1 week after surgery, the stitches are removed in the outpatient clinic, and the patient can walk normally without supporting the crutches, and the daily walking time is controlled within 1 hour. Knee mobility should reach more than 100 degrees. 6.Training at home in the second week after surgery: (1) Mobility training 0-45-90-120 degrees, from the beginning of the second week after surgery to the end of the second week after surgery, the mobility gradually increases. (2) Plyometric training begins with static squatting at 45-80 degrees. (1) Start static squatting exercises for quadriceps strength. The method of static squatting exercises: the posture is the same as the crotch riding action in Chinese martial arts or becomes a standing pile action. The patient’s legs apart, the distance between the two feet is slightly wider than the shoulders, the body remains upright position, can not lean forward, at this time the knees began to bend squat. The bending angle of both knees varies according to the patient’s physical condition and muscle strength. If the patient’s physical condition is good, good thigh muscle strength, squatting angle can reach 90 ° flexion, if the patient’s body is not good and thigh muscle strength is weak, both knees can be lightly flexed, this time the half squat position is higher, the patient more effort, with the practice of muscle strength and then increase the angle of the knee flexion. According to their physical condition to decide the number of exercises. The general requirement is to practice 2 – 3 groups per day, each group of exercises 10-15 times, each exercise time varies from person to person, for example, good muscle strength, a static squat time can last 5 minutes, while muscle strength is weak, a squat can only 2 minutes, in short, regardless of muscle strength is good or bad, each time squatting until the legs shake, can not hold on, so that the exercise once. ② straight leg raise exercise quadriceps medial head muscle strength. Straight leg raise training: the straight leg raise exercise is: the patient lying on the bed, legs naturally straight, the affected limb in the knee joint straight state raised 15 °, maintain the raised 15 ° posture, until the weakness. Rest for 2 minutes and repeat again. Just complete 10-20 times a day. 7, 3 weeks after surgery -5 weeks: can be “special walking training”, this period, generally advocate slow or medium speed walking, fast walking to be as little as possible. 3 weeks after surgery daily walking distance is best in about 1000 meters, 4 weeks after surgery can be increased to 1500 meters, 5 weeks after surgery can exceed 2000 meters. However, it is important that the swelling of the knee joint does not increase significantly after walking. If the swelling of the knee joint increases significantly after walking according to the above-mentioned distance, please reduce the walking distance, first by 500 meters, and if the swelling no longer increases you can break according to the reduced distance and increase the distance by the next week. 8.After 6 weeks after surgery, you can start walking without limiting the distance and also resume all activities of daily living and work, and try jogging. If joint swelling increases after too much walking or running, such exercises should be postponed appropriately. For athletes, if the muscle strength recovers well at this time, they can start to participate in supplementary training and quickly overtake to regular special training. 9.After 9 weeks postoperatively, basically resume normal activities.
