Facial palsy, also called facial neuritis and facial nerve palsy, is a paralysis of the facial muscles. It is caused by a stroke of the facial nerve that innervates the facial muscles. The main manifestation is impaired facial muscle movement. The clinical manifestations of facial palsy mainly include inability to frown and frown, inability to close the eyes or incomplete photophobia, tearing and Bell’s phenomenon. The corners of the mouth are skewed to the healthy side, and the air leaks from the diseased side when puffing the cheeks, leaks from the diseased side when rinsing the mouth, drooling, and food stays between the teeth and cheeks on the diseased side when eating. In Chinese medicine, it is called crooked mouth and slanted eyes, commonly known as hanging line wind and aiming wind (just like a carpenter hanging a line and a soldier aiming). If the peripheral facial nerve palsy is of the same degree bilaterally, the face is expressionless, cannot smile, the eyes cannot be closed tightly, the lips cannot be closed tightly, food is retained in the cheeks when eating, and speech is slurred. In Chinese medicine, it is called generalized palsy, commonly known as vegetative palsy or silly palsy. Most of the causes of the disease are empty veins and veins, feeling wind and cold. The next cause is the deficiency of liver and kidney yin, and wind and yang disturbance. Again, the seven emotions are too extreme (joy, anger, sadness, thought, grief, fear, and fright), as well as damage to the internal capsule, intracranial hemorrhage, intracranial tumors, infections (such as meningitis), otogenic diseases (such as otitis media), tumors (such as auditory neuroma), trauma (such as bruises and falls), poisoning (such as alcohol), metabolic disorders (such as diabetes), nutritional deficiencies (such as B vitamins), immune disorders, vascular insufficiency, congenital facial nerve Nucleus pulposus hypoplasia and many other factors. In particular, the most common triggers of this disease are: ear pulling, tooth picking, alcohol consumption and sudden exposure to cold while sweating profusely. 1. Modern medical understanding of facial palsy. Western medicine classifies the disease into two categories: peripheral facial palsy and central facial palsy according to the location of the lesion. (1) Central facial nerve palsy: It refers to the facial nerve palsy caused when the lesion is located between the nucleus of the facial nerve and the center of the cerebral cortex, that is, when one side of the cortical brainstem bundle is damaged, also known as supranuclear facial nerve palsy (2) Peripheral facial nerve palsy: It refers to the facial palsy caused by the lesion of the motor fibers of the facial nerve (commonly known as crooked mouth). The lesion can be located below the facial nerve nucleus, such as the lower part of the cerebral bridge, middle ear or parotid gland. All the expression muscles on the side of the lesion are paralyzed, manifesting as inability to close the eyelids, inability to frown, cheek puffing and leakage, crooked mouth and eyes, and tearing eyes, etc. It may be accompanied by clinical features such as auditory changes, loss of taste in the first 2/3 of the tongue and salivary secretion disorder. Generally, facial palsy can be divided into three stages as follows: (1) Acute stage, within 1 week of onset. (1) Acute stage, within 1 week of onset. This is the period of progression of facial nerve inflammation and edema. (2) The recovery period, from 1 week to 3 months after the onset of the disease, is the key period for the treatment of facial palsy, and 95% of patients can recover during this period after acupuncture treatment. (3) The sequelae period, from 3 months to more than 6 months after the onset of the disease. There are three kinds of sequelae of facial palsy, such as facial muscle spasm, inverted error, and mouth-eye linkage. 3, how to do a good job of care for patients with facial nerve palsy (1) The acute phase should pay attention to keep warm. Apply hot and moist towels to the affected face 3-4 times a day for 15-20 minutes each time. Wear a mask when going out to avoid further wind and cold. (2) Oral antibiotics or Chinese medicine to clear heat and detoxification should be taken at the same time when there is pharyngeal infection. (3) Watch TV and computer as little as possible, which is not conducive to the recovery of the nerves. Since the eyes on the sick side of the disease cannot be closed, you should wear eye shields during sleep and when you go out, and apply antibiotic eye ointment to protect the cornea and prevent conjunctivitis. (4) Rinse your mouth after eating and remove the food residue between the affected side of the cheek and teeth to keep your mouth clean. Give nutritious, easy-to-digest soft food, prohibit smoking and alcohol, avoid eating irritating food, and do not eat cold food, especially not chilled food. (5) Those who take prednisone should strictly follow the doctor’s prescription, not to increase or decrease the dosage at will, and pay attention to any side effects. (6) Patients should frequently massage the paralyzed facial muscles to the mirror and perform the necessary expression muscle training for 3-4 times a day for 3-10 minutes each time.
