Differential diagnosis of tremulous myotonia, bradykinesia

       Tremor palsy, also known as Parkinson’s disease, is a common neurodegenerative disorder that develops after middle age and is characterized by resting tremor, muscle tonicity, and bradykinesia. It should be differentiated from the following diseases: Secondary tremor palsy syndrome 1. Cerebrovascular tremor palsy syndrome: It occurs after lacunar infarction or acute stroke, with hypertension, atherosclerosis, cone bundle sign, pseudobulbar palsy, etc. Cranial CT examination is helpful for diagnosis.  2, post-encephalitis tremor palsy syndrome: pre-morbid history of encephalitis, seen at any age, common motility crises (episodes of bilateral upward involuntary eye spasms), seborrhea, increased salivation.  3, pharmacogenic tremor palsy syndrome: a history of taking antipsychotics such as phenothiazines or rosemary-type antihypertensive drugs, which interfere with the metabolism of catecholamines in different links and cause the symptoms to disappear after stopping the drug.  4, toxic tremor paralysis syndrome: mainly based on the diagnosis of toxic diseases, such as a history of carbon monoxide poisoning before the disease.  The diagnosis is based on a history of poisoning, such as carbon monoxide poisoning before the disease. 1, idiopathic tremor: tremor similar to the disease, but no muscle tonicity and bradykinesia symptoms, may have a family history of benign course, a few may evolve into tremor paralysis.  2, senile tremor: seen in the elderly, tremor fine and fast, in the random movement, without muscle tonicity.  3, hysterical tremor: there are mental factors before the disease, the form, amplitude and speed of the tremor is variable, aggravated by concentration, and there are other manifestations of hysteria.  It is important to differentiate from some central nervous system degenerative diseases with tremor paralysis symptoms, such as hepatomegaly, primary upright hypotension, cerebellopontocerebellar olivary atrophy and so on. In addition to symptoms of tremor palsy, these diseases also have other neurological symptoms corresponding to each disease, such as cerebellar symptoms, pyramidal fasciculus, and oculomotor palsy.
