Advanced prostate cancer is not the “end of life”

  When it comes to prostate cancer, people often think that it is the “patent” of Europe and the United States, because the incidence rate of this disease in Europe and the United States is as high as more than 100/100,000, ranking the second cause of cancer death. The incidence of prostate cancer in China is much lower than that of western countries, but it has been increasing significantly in recent years. According to statistics, there are about 80,000 new cases of prostate cancer in China every year, mainly occurring in middle-aged and elderly people over 60 years old. Because of the hidden location of the prostate gland, the cancerous area mostly occurs around the posterior lobe, and the early stage does not cause difficulty in urination due to compression of the urethra, so there are often no symptoms in the early and middle stages of the disease, which is difficult to arouse people’s vigilance; even if there is some discomfort, such as difficulty in urination, it is often mistaken as a sign of old age or due to prostate hypertrophy, which delays early diagnosis and treatment.  1. Who do prostate cancer like to favor?  The prostate is a gland located below the bladder and wrapped around the urethra, normally the size of a chestnut, with the urethra passing through the middle, unique to men. The fluid it secretes is a component of semen. The true cause and pathogenesis of prostate cancer is not well understood. At present, it is believed that prostate cancer occurs in the following five types of people: (1) Older men. It is now known that the older men are, the higher the chance of prostate cancer, especially those around 70 to 80 years old. This is because men’s glands change according to their age, and after the age of 50, men’s glands tend to proliferate.  (2) Men who do not pay attention to their work and rest. The male organism produces a melatonin, a hormone that inhibits the proliferation of prostate cancer cells. Some men often stay up late because of their work or night life, which can lead to endocrine disorders and reduced secretion of melatonin hormone, making them prone to prostate cancer.  The most important thing is that you should be able to get a good idea of what you are doing.  The actual fact is that you can find a lot of people who like to smoke and drink. The nicotine and coke in cigarettes can have an effect on the body and can easily induce prostate cancer. The actual fact is that you can find a lot of people who have been in the business for a long time, and that’s why you should be able to get a good deal on your own. The actual fact that some men love to drink carbonated beverages, strong tea, etc., also increases the risk of prostate cancer.  (5) People who are sexually confused. Some men who are sexually active have a high level of androgen secretion, which leads to hormonal imbalance in the body and can easily lead to prostate cancer. Some men have sex too early or have too many partners, making their bodies too androgenic and increasing the chance of infection, which can lead to prostate cancer.  2. What are the symptoms of prostate cancer?  There are no symptoms in the early stages of prostate cancer. When the tumor enlarges to the extent of blocking the urinary tract, symptoms similar to those of prostate hyperplasia will appear, such as slow urine flow, frequent urination, urgent urination, interruption of urine flow, incomplete urination, difficult urination and even incontinence, but hematuria is not common, and in the late stage, back pain, leg pain, anemia, swelling of lower limbs, bone pain, pathological fracture, paraplegia, difficult urination, oliguria, anuria and uremia. Since prostate cancer is asymptomatic in the early stage, even if there is discomfort, it is not enough to attract the patient’s attention, while benign prostate enlargement and prostatitis produce symptoms similar to cancer, so it is difficult to distinguish them in the early stage. When the tumor enlarges to a certain extent and puts pressure on the urethra, it is often not early. It can be seen that early detection of prostate cancer is very important. Especially for patients with prostatitis and prostate hypertrophy, if the disease is recurrent and does not heal, close attention should be paid to the changes of the disease to prevent cancer.  3.How to detect prostate cancer early?  Prostate cancer is not terrible, early detection and timely treatment can be cured without affecting life expectancy. This shows that early diagnosis of prostate cancer is very important. Prostate cancer screening mainly includes transrectal finger examination, serum prostate specific antigen (PSA) measurement and transrectal ultrasound examination in three steps. In general, men over the age of 50 should have an annual examination; when abnormalities are found, transrectal ultrasonography and ultrasound-guided prostate puncture biopsy are then performed.  4.How to prevent and treat prostate cancer?  For early stage prostate cancer, the best treatment is to do radical surgery, that is, when the cancer is confined to the prostate, it will be radically removed, which can achieve lifelong cure and does not affect the patient’s quality of life. For prostate cancer that penetrates the prostate envelope and has local invasion, radical surgery or debulking surgery can be considered on the basis of neoadjuvant therapy and combined with endocrine therapy. With the improvement of surgical level and anatomical techniques, radical surgery that preserves sexual nerves and blood vessels can restore sexual function for most patients after surgery. Most of the advanced prostate cancers have good results after anti-androgen therapy and can be combined with local radiotherapy, chemotherapy or interventional therapy according to the situation. In clinical work, we often encounter patients who have undergone transurethral resection of prostate gland for prostatic hyperplasia, and after the surgery, when the pathological examination of the resected prostate tissue is performed, prostate cancer is accidentally found, which is called incidental cancer. This type of prostate cancer is mostly focal, with small tumor size, well-differentiated cells, slow growth, little chance of metastasis, and good prognosis.  To prevent prostate cancer, we should pay attention to the following points: (1) control animal fat in food, keep the fat content about 20% of total calories; (2) eat less red meat such as pig, cow and sheep as possible; (3) eat more soy products, vegetables, fruits and cereals, which contain phytoestrogens that can reduce the occurrence of prostate cancer. Especially soy products, you need to eat more than 50 grams per day; (4) drink more green tea, which can reduce the incidence of prostate cancer. In addition, Dr. Brian Moran, an oncologist at the American Center for Prostate Cancer Research, recently found that eating more fish and tomatoes has a role in preventing and reducing the incidence of prostate cancer.  How to establish the new concept of “survival with tumor”?  The classical tumor treatment model – the concept of complete killing of tumor, that is, the tumor must be eradicated, even if the old capital is lost. Today, with the emphasis on holistic medicine, it has shown its drawbacks, which often results in overtreatment of tumors. In the past, it was believed that the only condition to prolong the survival of tumor patients was the degree of cancer-free that could be achieved by various treatments. In other words, the treatment of cancer must kill or remove the last cancer cell, for which extended radical surgery, intensive or shock chemotherapy, radical radiotherapy, etc. were once pursued. However, contrary to the wishes, so far, the highest efficacy that can be achieved by the above treatments is only clinical cure, and tumor recurrence and metastasis are still an insurmountable problem, and patients generally experience a decline in quality of survival after treatment, and even die because they cannot tolerate further treatment. In fact, for patients with incurable advanced cancer, the goal we pursue is to prolong life and improve quality of life as much as possible. As a result, Professor Schipper from Canada proposed in 1994 in a new paradigm about tumor treatment that effective treatment does not require complete regression of the tumor, and the response of the body is most important for cancer treatment. This view is consistent with the efficacy characteristic of Chinese medicine in treating malignant tumors – survival with tumor, and also shows that survival with tumor has its scientific nature. Sun Yan, a famous oncologist, also pointed out, “People are no longer satisfied with the situation where the tumor is cured and the patient becomes crippled or severely dysfunctional and thus lives a miserable life. The phenomenon of ‘the disease is cured and the person is disabled’ can no longer continue.”  With the development of medical research, survival with tumor has become a new concept of cancer recovery. In fact, people live with disease all their lives. There may not be a cure that people expect in the short term, but it may be more relevant to reduce the harm of cancer to a state of a chronic disease that people can tolerate. This is because the goal of treatment is to control and reduce the risk of cancer to life, not to completely eliminate cancer regardless of the human body. Currently, the World Health Organization and other international agencies have defined cancer as a chronic disease. In other words, treating cancer as a chronic disease like diabetes and hypertension is actually not as terrible as people think, as long as prevention, early detection and early treatment are strengthened. Let patients coexist with it safely for a long time to maximize the quality of life. Using modern medical technology to keep the cancer condition from progressing, maintain a stable level, and reduce its damage to the organism will probably be the most feasible treatment method. In conclusion, prostate cancer is also a chronic disease, and treatment should be moderate, not excessive, and too much is not enough. The most important thing is to take care of the immunity of the body to support the righteousness, and to combine work and rest.
