Abstract: The patient complained of symptoms such as urinary urgency, frequency and blood in urine, which were once thought to be caused by a urinary tract infection and were not relieved by self-administered medication, so he came to our hospital for consultation. After urological ultrasound examination, an occupancy in the bladder was found, after which the patient was convinced to undergo cystoscopy as well as pathological examination, which suggested adenocarcinoma of the bladder. After the cancerous lesion was found, the patient was promptly subjected to radical cystectomy, which completely removed the location of the lesion and the patient recovered well.
Basic information】Male, 50 years old
Type of disease】Cancer of the bladder
Hospital】The First Affiliated Hospital of Zhejiang University School of Medicine
Consultation time】February 2021
Treatment plan】Surgical treatment (radical cystectomy) + medication (cefixime capsule)
Treatment Period】5 days of hospitalization
Treatment effect] The tumor was completely removed and the patient recovered well.
I. Initial consultation
The 50-year-old patient, Mr. Li, came to our hospital because his symptoms of urinary frequency, urinary urgency and painful urination were aggravated. The patient reported that he was usually healthy and had no basic diseases such as hypertension. Treatment. However, after taking the medicine, the patient’s symptoms did not change, but had a tendency to worsen, so he wanted to come to our hospital to check the specific cause. After understanding the patient’s symptoms and medical history, the patient underwent urological ultrasound examination, which indicated strong echogenicity in the bladder and suspected the presence of a tumor, and then the patient was allowed to undergo cystoscopy as well as pathological examination, which indicated that it was adenocarcinoma of the bladder. Since the patient was in the early stage of cancer and the disease had not yet spread, radical cystectomy was required for radical treatment.
II. Treatment process
After explaining the condition clearly to the family and the patient himself, the patient’s family was informed of the need to perform radical cystectomy on the patient, and after obtaining the consent of the patient and his family, we scheduled the surgery for the patient as soon as possible. Before the surgery, the patient was given blood pressure, routine blood tests, coagulation tests, etc., and contraindications to the surgery were excluded. Postoperatively, the patient was given etimesine sulfate infusion to prevent postoperative infection. After 5 days of hospitalization, the patient recovered well from the surgical wound, and there was no blood leakage or exudate. The patient’s symptoms such as urinary frequency and painful urination were relieved, and he was discharged from the hospital and given cefixime capsules to prevent infection, and the patient was asked to come for a review 2 weeks after discharge.
III. Treatment effect
The location of the lesion was successfully resected during the operation, and no tumor metastasis was found intraoperatively. The bleeding was relatively low and no serious complications occurred. After the surgery, the patient’s symptoms such as urinary frequency, urinary urgency, urinary pain and blood in urine disappeared, heart rate was normal, blood pressure was stable, all vital signs were stable, the surgical incision recovered well, the patient was able to eat and drink normally, and defecation and urination were good. Five days after the operation, the patient had no symptoms of postoperative infection such as fever and other uncomfortable symptoms, and all the above symptoms disappeared, so he was discharged from the hospital. At the follow-up examination 2 weeks after discharge, the patient reported that he was in good physical condition, no obvious abnormalities were found in the examination, and he was able to remove the urinary catheter normally.
IV. Notes
We are glad that the tumor was found in time and we performed surgical resection on the patient in time. The patient recovered well after surgery and was discharged successfully. However, since the patient’s wound had not completely healed at the time of discharge, the patient still needs to pay attention to the following elements in daily life.
1, daily life should pay attention to a light diet, as little as possible to eat or not eat spicy and stimulating food, so as not to affect the recovery of the surgical incision; it is recommended that the patient should rest for 1-2 months after discharge, can be appropriate to carry out activities, but should avoid excessive exercise and excessive fatigue, especially not to stay up late, to prevent the reversal of day and night.
2, after discharge from the hospital need to comply with medical advice to take anti-infective drugs, can not stop drugs on their own, so as not to cause infection. If there are symptoms such as cloudy urine and blood in the urine, you should seek medical attention promptly.
3. Pay attention to keep a good attitude, don’t scare yourself because it is cancer and feel hopeless, you should trust doctors and actively cooperate with them to prolong the survival time.
V. Personal insight
In the early stage of the disease, patients may not have any symptoms of bladder adenocarcinoma, and when symptoms appear, patients often mistake it as a case of urinary tract infection and therefore ignore the treatment, which may lead to serious consequences. But actually both simple urinary tract infection and bladder adenocarcinoma should be treated properly. If it is a urinary tract infection that is not treated promptly and properly, the patient may develop sepsis. If it is a bladder adenocarcinoma that is not treated promptly, the patient may develop tumor metastasis. The patient in this article also neglected treatment in the early stage of the disease, which led to further aggravation, but fortunately, the patient was found in time and treatment was not delayed for too long, so the overall prognosis is good. In addition, the patient’s good recovery was also due to the high compliance and active cooperation in treatment. Therefore, when there is discomfort, timely medical consultation to identify the cause of the disease and active cooperation with treatment will be more beneficial to ensure a good prognosis.
