Be alert to precancerous lesions – gastric polyps

  Many patients may find polyps in their stomach by chance during gastroscopy. These polyps may be large or small, with or without a tip, smooth or not, papillary or lobulated, and may have different shapes. Do they need to be operated? How to completely treat it?  A. What is a polyp Gastric polyp is a benign elevated lesion that is limited to the gastric mucosa. In the early stage, there are no symptoms. When symptoms appear, they are often manifested as hidden pain in the upper abdomen, bloating, discomfort, a few may appear nausea, vomiting, stomach acidity, anorexia, indigestion, weight loss, diarrhea.  Second, what are the dangers of polyps Polyps are also known as precancerous lesions, which have a high cancer rate. Pathologically, gastric polyps are divided into 2 categories: (1) proliferative or regenerative polyps: this type of polyps account for about 75% to 90% of gastric polyps, is the formation of inflammatory mucosal hyperplasia polyp-like material, not a real tumor. The general diameter is less than 1.5cm, round or olive-shaped, with or without a tip, smooth surface, can be accompanied by erosion. A small number of hyperplastic polyps can become heterogeneous or adenomatous and produce malignant changes, but their cancer rate is generally not more than 1%-2%.  (2) Adenomatous polyp: It is a benign gastric tumor derived from the epithelium of gastric mucosa, accounting for about 10%-25% of gastric polyps. Generally larger, spherical or hemispherical, this type of polyps with a high rate of cancer, up to 30% ~ 58.3%, especially tumor diameter greater than 2cm, villous adenoma, heterogeneous hyperplasia Ⅲ higher rate of malignant transformation.  Third, how to treat polyps Hyperplastic polyps are non-neoplastic polyps, because no malignant lesions will occur, and the effect is better with symptomatic treatment by internal medicine. Adenomatous polyps cancer rate can be 30% to 58.3%, the biopsy confirmed the diagnosis should be surgical treatment, that is, the earlier the discovery, the earlier should be removed. The most simple, effective and less invasive treatment method is endoscopic excision. The more commonly used are high-frequency electrocoagulation resection method and argon ion coagulation.  Fourth, the polyp can be removed once and for all?  Obviously not, in general, polyps and personal physical and lifestyle habits have a certain correlation, once the polyps are found, the recurrence rate is still very high within a certain period of time after removal. Therefore, patients who have been found to have gastric polyps, it is best to review the gastroscopy within one or two years.
