In 1984, Prof. Ye first proposed the principles of hormone use in patients with renal syndrome in China, and supplemented them in 1990 by proposing three major guidelines for the use of hormones, i.e., the initial dose should be adequate, the dose should be reduced slowly, and the maintenance time should be long. The use of cytotoxic drugs in patients with renal syndrome was also proposed, thus improving the efficacy of treatment for renal syndrome. However, side effects are still inevitable. In order to minimize the side effects of drugs, improve the efficacy and reduce recurrence, Professor Ye has developed a set of mature experience by combining Chinese and Western medicine. 1, the first treatment phase of the initial cases of the first hormone amount should be large enough to induce rapid remission of renal syndrome. The amount of prednisone should be 1mg/kg/day for adults and about 2mg/kg/day for children. The younger the patient, the higher the dosage, but it should not exceed 80 mg/day. if the patient has reduced liver function, the patient should be switched to methylprednisolone at the same dosage as prednisone. Professor Ye believes that it is better to take prednisone once in the early morning, because this is in line with the circadian regularity of corticosteroid secretion, thus reducing the adverse effects of hormones. The first treatment phase must be used for 8 weeks to ensure better efficacy and reduce the recurrence of renal syndrome. However, sometimes the duration of medication for children can be shorter, about 4 weeks or more, or for adults, after less than 8 weeks of medication, the dosage can be gradually reduced after 2 weeks of remission, but the time should not be limited, depending on the results of the disease. At the stage of using large doses of hormones for the first time, since hormones are masculine products and are taken for a long time, side effects of one kind or another are still inevitable. At this stage, Chinese medicine often identifies the evidence of yin deficiency and fire exuberance, so the treatment should be to nourish yin and lower fire. The combination of Chinese and Western medicine can not only reduce the side effects of large doses of hormones, but also improve the immunity of the body, thus increasing the remission rate. Prof. Ye also made his own kidney special capsule, whose main drugs are dry lotus herb, chasteberry, salvia, motherwort and jinyinhua, so as to achieve the function of nourishing yin and lowering fire. 2.The dosage should be reduced after 8 weeks of high-dose hormone treatment, and the dosage should be reduced by 10% of the original daily dose every 2 weeks, generally 5mg every 2 weeks for adults. If there is partial remission (proteinuria is less than 3g/day or reduced by more than half, edema and other symptoms are reduced), the dose should be reduced to a small dose (0.5mg/kg/day for adults and about 1mg/kg/day for children), and then the dose should be changed to 2 days’ dose and taken once every other morning, which can be a longer-term continuous treatment. The dose should be changed to a 2-day dose, once every other morning, for longer-term continuous treatment, and then slowly reduced depending on the condition. At that time, the main immunosuppressant is CTX, usually 0.2g in 20ml of saline, intravenous injection, once every other day or 8-12mg/kg, intravenous drip, for 2 days, repeated semi-monthly, the total cumulative amount is less than 150mg/kg. if the first treatment phase has been completely remitted, can continue to reduce the dose, but should emphasize very slowly, the smaller the dose, the slower the reduction, so as to reduce the recurrence of renal syndrome. The smaller the dose, the slower the reduction, so as to reduce the recurrence of renal syndrome, because too rapid reduction of hormone may cause rebound phenomenon or corticosteroid withdrawal syndrome. Although the dose is slowly reduced due to individual differences or other reasons, sometimes the rebound phenomenon may occur. At this stage, due to the withdrawal of hormone, the patient often loses yin and yang, and is deficient in both yin and yang, so it is appropriate to treat both yin and yang, and the formula is Jin Kui Kidney Qi Pill plus or minus. Prof. Ye believes that adding yang tonic with the gradual reduction of hormone can help reduce the body’s dependence on hormone and prevent rebound; it can antagonize the feedback inhibition of exogenous hormone and prevent the emergence of corticosteroid withdrawal syndrome; it can regulate the function of hypothalamus-pituitary-adrenal It also has the function of regulating the hypothalamic-pituitary-adrenal axis, adjusting the immune disorder of renal syndrome, and also plays the role of preventing infection. 3.The first high-dose treatment can be reduced to a small dose (1mg/kg every other day for adults and 2mg/kg every other day for children) for 8 months or longer, and then reduced to maintenance dose (i.e. 0.4mg/kg every other day in the early morning) according to the above method, which is the physiological dose and rarely has adverse effects. If complete remission is obtained in the small dose continuous treatment, then take the original amount for another 4 weeks, then slowly and regularly reduce the amount to the maintenance amount, and gradually reduce the amount to stop after a period of discretionary maintenance depending on the condition. At this time, Chinese medicine with the treatment, the disease due to the lingering, the loss of vital energy, spleen and kidney Qi deficiency, kidney deficiency can not fix astringency, spleen deficiency will be in the spleen Qi down, so the essence of the injection of protein urine, so the treatment should be on the basis of nourishing Yin, the appropriate addition of beneficial Qi, kidney consolidation of the Chinese medicine, such as the choice of Right Return Pill combined with Ginseng Ling Baijiao San plus reduction, Qi deficiency is significant without hypertension, hematuria, heavy use of Astragalus (30-60g); there are heat toxicity see If the condition requires the addition of CTX, because the drug is an immunosuppressant, it is easy to damage the kidney yang, so it is advisable to use warm kidney medicine (such as Xian Ling Spleen 12g, Baji Tian 12g, Cuscuta sinensis 12g) to improve the efficacy, and because it has the side effects of bone marrow suppression, white blood cell drop or platelet reduction, so add blood and qi tonification as appropriate. Chinese herbs, such as Angelica Sinensis 12g, Radix et Rhizoma Polygonati 10g, Rhizoma Polygonati 9g, Radix et Rhizoma Polygonati 12g, etc. In case of gastrointestinal reactions, Chen Pi 9g, Radix et Rhizoma Pinelliae 6g, Rhizoma Polygonati 9g, Radix et Rhizoma Polygonati 9g, etc. may be added. During the hormone maintenance treatment phase and after discontinuation in order to consolidate the therapeutic effect, Professor Ye formulated his own renal special kang capsule (main drugs Radix et Rhizoma Ginseng, Radix Astragali, Rhizoma Atractylodis Macrocephalae, Radix Bupleuri, Fructus Lycii, Radix Angelicae Sinensis, Radix Paeoniae Alba, etc.) in order to achieve the function of tonifying Qi and benefiting the kidney. Cases where complete remission has been achieved with initial treatment can usually be reduced to maintenance doses as described above, usually taken for a period and then slowly reduced to discontinuation. In order to prevent the recurrence of renal syndrome, it is advisable to tonify the kidney and strengthen the spleen, according to Prof. Ye, adults should focus on tonifying the kidney by adding and subtracting Liu Wei Di Huang Tang, and children should focus on tonifying the spleen by adding and subtracting Si Jun Zi Tang. In some patients, although complete remission is achieved in the first treatment, but relapse within a short period of time (<6 months), or even relapse when the dosage is reduced to a certain level (i.e. hormone-dependent), hormone therapy can be reintroduced and a course of CTX can be added, and when the hormone dosage is reduced to the maintenance dosage according to the above routine, the drug can be continued for 12 to 18 months. When using high-dose hormone and CTX, the treatment rules of TCM are the same as above. During the maintenance treatment phase of hormone dose, the focus is on benefiting Qi, tonifying kidney and strengthening spleen. The formula is the same as above. After remission of renal syndrome, the main objective is to consolidate the therapeutic effect and prevent recurrence. There are many recurrence factors, but the most important one is infection, especially cold. According to Chinese medicine, the lung is the main surface of the body and is the fence of the person. If the lung is deficient in qi, it is easy to feel external evil, so the lung should be tonified to benefit qi and consolidate the surface. These drugs have the ability to improve the immune function of the body, enhance the function of the reticuloendothelial system, and promote the body's production of interferon. Professor Ye often uses this formula in combination with herbs for strengthening the spleen and warming the kidneys to treat the lungs, spleen and kidneys together, which is helpful in preventing colds, consolidating the therapeutic effect and reducing recurrence. Some patients with renal syndrome cannot be relieved despite the active use of standard hormones and cytotoxic drugs, and often the edema does not go away, which is a refractory renal syndrome. This is mainly due to the damage to yin and yang after a long period of time. Therefore, we should use the method of nourishing kidney yin, warming kidney yang, promoting water and reducing swelling, and choose Ji Sheng Kidney Qi Pill plus or minus. In patients with nephrotic syndrome, long-standing disease enters the ligament, stasis of blood, mostly in a high coagulation, high viscosity state, then many patients have clinical dull pain in the waist, dark face, dark lips, dark tongue or purple spots, that is, there is blood stasis witnessed, so in each stage of hormone therapy using Chinese medicine should be added to activate blood circulation and remove blood stasis, such as leech 10g, Dan Shen 30g, saffron (or Chuan saffron) 5g, Chuan Xiong 12g, Angelica sinensis 9g, Red Peony 9g, Yimu Cao, and the other two are used in the treatment. Red peony 9g, motherwort 15g, whole worm 9g, ground dragon 9g, etc. If the patient has elevated blood lipids, Hawthorn or single-flavored Hawthorn decoction can be added and taken frequently, which can lower blood lipids and alleviate hypercoagulation and hyperviscosity, so that the stasis of blood can be reduced. In addition, peach kernel should not be used to activate blood circulation and remove blood stasis after renal failure in patients with renal syndrome.
