In clinical practice, we find more and more patients with particularly sensitive facial skin, mainly manifesting as slightly red, irritable and highly sensitive face, and in severe cases, capillary dilation, erythema and papules. And the disease is prolonged, sometimes mild and sometimes severe, often accompanied by the patient’s agitation, depression and lack of self-confidence. Clinicians are often given the diagnosis of sensitive skin or facial dermatitis and treat the symptoms hastily.
If we take a careful medical history and analyze the causes, we will find that most of them belong to hormone dependent dermatitis.
When I discussed this disease with my colleagues in the industry, dermatologists generally agreed that the incidence of this disease is high. And the misdiagnosis rate is very high. In the past, many of those clinically diagnosed as sensitive skin, facial dermatitis, facial seasonal dermatitis, etc. were hormone-dependent dermatitis, and the misdiagnosis rate reached 76% due to the clinicians’ lack of awareness, attention and responsibility.
76%. In addition, this disease is very harmful. The state of hormone dependence on the face often lasts for years or even decades, and the skin changes from sensitive discomfort to erythema, papules, and hair expansion;
So what is hormone-dependent dermatitis?
Glucocorticoid-dependent dermatitis refers to the sudden discontinuation of glucocorticoid-containing preparations after long-term topical use, resulting in the recurrence and aggravation of the original skin disease, forcing patients to use glucocorticoids.
What are the causes of hormone-dependent dermatitis?
The causes of hormone-dependent dermatitis are generally divided into hormonal drug abuse and adverse cosmetic reactions. Long-term improper topical glucocorticoid use is the main reason for the formation of hormone-dependent dermatitis, including:
① wrong choice of hormone preparations.
② Excessive duration of medication, experts believe that the use of high potency G hormones for > 20 days and low and medium potency hormones for > 2 months; it is possible to form hormone dependent dermatitis.
③ Abuse, long-term use of cosmetics containing hormones or unknown ingredients, capillary dilatation, acne-like rash, fine hair and other symptoms aggravated after stopping use.
How to break hormone dependent dermatitis
Hormone-dependent dermatitis itself is prone to recurring attacks. If you think this recurring phenomenon is the skin detoxifying, you are very wrong; the inflammation of this attack needs to be controlled with medication. Through proper treatment and care, application of different hormone replacement drugs such as oral and topical, combined with herbal conditioning, most patients can receive good results.
It is mainly done in two stages.
Firstly, the disease is associated with many bad cosmetics, so firstly, the cosmetics should be stopped. Then the treatment is carried out in stages.
I. Episodic stage
There is an obvious rash, when the face is red, swollen, dry, itchy and symptomatic, take the cold compress method. If possible, cold spray treatment can be used for better results. At this stage it is necessary to use oral medication to control, symptoms. It also needs to be applied together with topical medications. Oral is simply an anti-allergy drug, plus anti-inflammatory drugs. You can choose to use Desloratadine with Mennen tablets. If the effect is not good, you can consider with hydroxychloroquine oral. Chinese medicine also has a very good effect on this. The main thing is to clear the heat and detoxify the toxins and cool the blood. If the skin is red and swollen with oozing, use cold and wet compresses. Cold wet compress can be used saline refrigerator cold, wet compress face, multiple times a day. Generally three to five days, the redness and swelling will subside. In the cold compress at the same time, with tacrolimus ointment topical application. After one week, the symptoms will basically subside. The tacrolimus ointment will be reduced once every 1 to 2 weeks, every other day for 2 days! Skin moisturizer needs to be used throughout the process, unless there is oozing when no moisturizer is used.
A Chinese herbal remedy for wet compresses is recommended: 50 grams of honeysuckle and 20 grams of raw licorice, decocted and cooled and applied cold.
During the attack period, it is recommended to visit a hospital dermatology clinic for face-to-face consultation and prescribe therapeutic drugs according to the specific situation.
Second, the second is the care of the rash after it subsides
Moisturizing is a long, long process and must be done to take care of the skin, pay attention to sun protection and reduce all kinds of external skin irritation. Apply moisturizer several times a day to keep the skin from drying out.
During this period, you can find an experienced herbalist to prescribe some Chinese medicine for physical conditioning. Or take some oral medications to improve immunity, such as transfer factor capsule, mushroom mushroom polysaccharide and so on. By improving your own constitution, you can achieve the purpose of avoiding recurrence.
To sum up, the treatment process of this disease is slow, so you should have enough confidence and perseverance and persistence.
