Pericardial effusion is a relatively common symptom in oncology, mostly caused by secondary metastasis, and rarely seen in primary tumors of the pericardium. The first tumor that is prone to pericardial effusion is lymphoma, followed by small cell lung cancer, lung adenocarcinoma and breast cancer, and the metastasis of cancer cells is mostly through hematogenous metastasis to the pericardium. This is well explained by the theory of Chinese medicine, that is, the so-called pericardium “receives evil on behalf of the heart”. Early detection, early diagnosis and early intervention of pericardial effusion should be achieved. However, it is easy to be misdiagnosed clinically. Once there is tachycardia, uncorrectable shortness of breath, and unexplained abnormal liver function, one should be alert to whether it is caused by pericardial effusion. The symptoms are related to the amount and growth rate of the pericardial effusion. A large volume of effusion, more than 2 cm, or rapid growth will result in clinical symptoms; while in some cases, the growth is slow and the patient may have no obvious symptoms even if the volume of effusion reaches 3 cm. How to deal with it clinically? The first method commonly used in Western medicine is pericardial effusion drainage. When a large amount of pericardial effusion is accompanied by pericardial filling, the pericardial effusion should be drained immediately to relieve the burden on the heart and to avoid death from acute heart failure. The second is pericardial windowing, which is a surgical procedure to find the lowest point of the pericardium, cut a cross, and drain the effusion into the chest cavity. The above two methods can effectively relieve the symptoms, but there are some defects, pericardiocentesis placement is easy to cause infection, and the more fluid is pumped, it is impossible to remove the tube; pericardial opening may lead to coronary artery injury, pulmonary and incisional complications. Chinese medicine treatment, the first is herbal soup, the pathogenesis of which is mainly deficiency of heart yang and water drink upward, when warming heart yang and warming water drink, using Gui Zhi Gan Cao Tang or Zhen Wu Tang, adding Draban Qian Zi. In the past, the textbook overemphasized Scapularis for the treatment of hanging drinks (such as chest water). In fact, Scapularis is useful for all edema in the body, and it has good effect on pericardial effusion and ascites, but the amount should be large, I usually use 30g in clinical practice. Secondly, moxibustion for Xueli is the earliest method I have worked out in clinical practice. It is also known as the great luo of the stomach, which is the place where the inhaled clear qi and the qi of water and grain converge, and the xu li is exactly the location where the pericardium is transmitted to the body surface. In the treatment of pericardial effusion, special attention should be paid to avoid using cold drugs and diet, I have had a painful lesson. A patient with lung cancer, who had a medium amount of pericardial effusion because of insisting on moxibustion, was re-examined as a small amount of pericardial effusion after the whole department discussed and recommended chemotherapy with docetaxel. However, the patient’s lover still insisted on moxibustion for Xueli every day, and the amount of fluid drained decreased day by day, and finally she was discharged from the hospital. In addition, there is an esophageal sinus point next to Hollow Lane, also called Life Pass (a life-sustaining pass), which belongs to the Spleen meridian, in the lateral part of the chest, in the 5th rib space, 6 inches from the front median line, and is a particularly good point for restoring the body,. I used to consult in a hospital in Beijing where the patient was very weak in the very late stage (lung and liver bone metastasis from intestinal cancer, huge ulcerated surface in the sacral vertebrae area), with a heart rate of 140-160 beats per minute, and moxibustion of this point with Xueli extended her life for 1 year. There is another explanation for this point: the esophageal sinus is also related to the esophagus, so it can treat all the diseases of the esophagus.
