At present, we often use in clinical microsculpture plastic surgery are the following: 1, facial autologous fat transfer microsculpture Autologous fat transfer (also known as fat injection, fat filling) is one of the increasingly sought-after procedures. The reason why it is called micro-sculpting is because facial fat grafting only needs to take a very small needle eye in a hidden area can correct a variety of facial problems, such as narrow and flat forehead can become wide and full through fat grafting; temporal temples (temporal) and cheeks (cheeks) sunken, looks like a skeleton face, looks sharp and old, through fat grafting to restore the temples and cheeks round and full, face shape into a young oval Fat transplantation can make the apples of the cheeks (also called apple muscle and cheek and jaw junction area) look old and depressed, but fat transplantation can make the apples plump and protrude, showing a young and energetic appearance. Lip fat transplantation can make these furrows shallow and rejuvenate the face; thin and sagging lips look tricky and lack of femininity; lip fat transplantation can make the lips full and lightly lift up, highlighting the feminine sexy features; mildly low nasal bridge, rounded or receding chin can also be corrected by fat transplantation, eliminating the pain of implanting prosthesis. However, the current international and domestic technology cannot make the fat transplantation 100% viable, and some patients may not be 100% satisfied with one fat transplantation and need 2-3 injections to achieve better results, but once the injected fat is viable, it will exist permanently and the surgical effect will last. In a word, fat grafting has less damage, less complications, less damage to the donor area where the fat is obtained, and no immune rejection after grafting, so it can be a real microsculpture and is recommended. 2, facial hyaluronic acid injection microsculpture Hyaluronic acid, also known as hyaluronic acid, its basic structure is a large polysaccharide substance composed of two disaccharide units D-glucuronic acid and N-acetyl glucosamine, with molecular weight from 5,000 to 20 million. Its reticulated molecular structure can carry large amounts of water. There is a large amount of hyaluronic acid in the intercellular space of human skin tissue, which can absorb and preserve a large amount of water and provide a nourishing internal environment for cell proliferation and metabolism. As the human body ages, hyaluronic acid gradually decreases and the water content in the tissues decreases, which is reflected in the skin as sagging, dry flat, wrinkled, dry and dull skin. Therefore, injecting hyaluronic acid into the subcutaneous soft tissues can increase the water content of the tissues and make the soft tissues of the skin full and moist. Using this principle, we can improve all wrinkles (horizontal lines on the forehead, sunken or Kawasaki lines between the eyebrows, crow’s feet at the outer corners of the eyes, tear troughs on the lower eyelids, sunken nasolabial folds (decency lines), anterior jaw furrows (marionette lines), vertical longitudinal lines on the lips, and lip and jaw furrows) by injecting hyaluronic acid, and can perform lip augmentation, nose augmentation, chin augmentation, forehead augmentation, and temple filling, etc. It can be said that all areas requiring soft tissue It can be used in all areas where soft tissue augmentation is needed. Hyaluronic acid injection is also a veritable microsculpture because only a very small needle eye remains when injecting hyaluronic acid. However, the fatal weakness of hyaluronic acid injections is that hyaluronic acid is gradually and completely absorbed in the body, and the absorption time of current commercially available products is about 6-8 months, and reinjection is required after absorption. Compared with autologous fat, it is more expensive. 3, facial fat aspiration microsculpture Fat aspiration began to be applied in the 1980s, and has a history of more than 30 years, its technology has been very mature and widely used in clinical practice. However, in the traditional concept, fat aspiration is used for the body or facial obese parts or areas with significant fat accumulation to achieve the purpose of weight loss. However, we are now using fat aspiration for facial micro-sculpting, which means that it is not used for plasticity of the bloated and obese face, but for trimming one or several slightly bulging or relatively prominent areas of the face to make the overall contour of the face smooth, rounded and rejuvenated. Sometimes it can also be combined with fat transplantation to achieve the purpose of “fat transfer” to make the facial structure more perfect and more youthful. 4.Facial botulinum toxin injection microsculpture Botulinum toxin can block the connection between motor nerves and muscles, so that the muscles are in a non-motorized “relaxed” state, so selective facial expression muscle injection of botulinum toxin can reduce the activity of the expression muscle, the facial motor wrinkles (expression lines) disappear. For this reason, facial Botox injections are widely used in the medical aesthetic industry. Motor wrinkles (wrinkles that are obvious only when the facial expression muscles are in motion), such as horizontal lines on the forehead, sunken or Kawasaki lines between the eyebrows, crow’s feet at the outer corners of the eyes, and sunken nasolabial folds (decency lines), can be reduced or eliminated by Botox injections; and enlarged bite muscles (wide face underneath, Chinese character face) can also be improved by Botox injections. Currently, our center has a new mission for Botox. We combine Botox with fat grafting, and inject Botox with fat particles between the eyebrows, outer eyebrows, nasolabial folds, lips, and chin, in order to inhibit the activity of the expression muscles in these areas and prevent fat displacement to promote the survival of the fat, and several clinical cases have been followed up with good results. Botox injections are also truly minimally invasive, leaving no traces, and recovery is quick. The effect starts to appear 3 days after the injection, and after 3-6 months Botox is metabolized by decomposition and its effect then disappears. Therefore, Botox injections cannot be used once and for all, and are characterized by “short (short duration of effect), frequent (repeated injections are needed) and fast (fast surgery and fast onset of effect)”. It needs to be injected 1-2 times a year, but because it can delay the appearance of permanent wrinkles (age-related or gravitational wrinkles), its application is very promising.
