Continuous and clear verbal hallucinations are of great importance to the diagnosis of schizophrenia, but for children and adolescents, I personally believe that the diagnostic significance of their hallucinations is not as good as that of adults, and their diagnostic value will be relatively lower the younger they are. Children and adolescents’ brain, especially the rational brain is not yet mature, their rational thinking ability, rational control ability and rational expression ability are still developing, still at a low level, the actual diagnostic significance of their “phantom hearing” is also relatively small. Note: The rational brain here refers to the frontal lobe, the prefrontal lobe is responsible for the overall command of the brain, mainly plays the role of analysis, thinking, logical reasoning, judgment, awareness, security, coordination, control and so on. The prefrontal lobe is the latest development of all the structures of the brain, to the age of 2-3 years old before the official start of development, to the end of puberty before the basic formation, and fully mature women to 24, 25 years old, men are generally around 30 years old. This brain tissue is the most vulnerable of all the structures in the brain, and schizophrenia may be primarily due to structural damage and functional impairment of this brain tissue. Since the rational brain of children and adolescents is not yet mature, it is appropriate to take a cautious approach to the hallucinations and delusions they express, as well as to the diagnosis of schizophrenia, and to closely observe how these symptoms change and develop later on, especially in response to different treatments. Note: The above personal clinical experiences may not be correct and are for reference only. For diagnosis of specific cases, more consultation should be made with the patient’s attending physician who has a comprehensive and in-depth understanding of the patient’s condition.
