The doctor took some earthy yellow Chinese medicine powder to him and told him to abstain from meat. I went through his medical records and found that the 24-hour protein quantification was as much as 10 grams, with only 18 grams of albumin per liter of blood. The patient already had less protein in his blood, and with the abstinence from meat (which is the main source of protein) in his diet, it was no wonder that the patient’s edema was getting worse. I advised him to be admitted to the hospital and stay there and change to a combination of Chinese and Western medicine. He has improved and the edema has subsided. Fortunately, the treatment course was changed earlier, otherwise it would have been life-threatening. This incident illustrates two issues, the first is not to casually find unqualified “doctors” to see, these people are generally able to talk, or alarmist, or declare that the package of medical, so that you are deceived, “quack doctor kill”, the ancient teaching, do not hand over your life to the hands of charlatans; on the other hand, this incident also reflects the public’s lack of knowledge of chronic kidney disease food therapy.  Patients with nephrotic syndrome lose a lot of protein from the urine every day, which inevitably causes a lack of protein in the body over time, and if they do not eat more protein supplements, they will become more and more deficient. It is important to know that protein is the source of life, the lack of protein in the body, not only will edema, and will be a hundred diseases, the body to win thin, life-threatening. Therefore, these patients should not listen to the social rumors, kidney disease patients should abstain from meat, eggs, scaly fish and other unscientific claims, on the contrary, these patients should eat more of these proteins, especially foods rich in high-quality protein, such as milk, eggs, lean meat, chicken, fish and so on. By the way, many patients see both Western medicine and Chinese medicine, the two doctors sometimes have different instructions, do not know which one to listen to; my opinion, Chinese and Western medicine have their own strengths, the “combination of Chinese and Western medicine” can complement each other’s strengths, which is the best policy, but if Chinese and Western medicine without consultation, and each of their own way, it is possible to run counter to each other, cancel each other out, this is not a “combination”, but a “mixture of Chinese and Western medicine”. In general chronic kidney diseases, such as occult proteinuria, chronic glomerulonephritis, and chronic pyelonephritis, there can be no hard and fast rules in terms of protein intake. However, it should be noted that these patients should be examined by a doctor in about three months to six months and have their blood tested for elevated blood creatinine and urea nitrogen. At this critical time, dietary control of protein intake is a rather important part of treatment. In my doctor’s journal, there is a record of a young patient with azotemia, whose condition was quite stable under my treatment, and who was originally referred by my colleague for a kidney transplant, but I felt that he was still able to work as a light worker and that a kidney transplant would not be beneficial to him, so he was given conservative treatment. This patient is a peanut eater. On the night of the Mid-Autumn Festival, his wife cooked a big pot of peanuts for him to eat, and he couldn’t help but eat a lot of them at once, and the next morning he developed uremia. Gluttony ate a meal of peanuts, why will occur uremic syndrome? It turns out that eating too much protein will accelerate the kidney damage of chronic kidney disease, azotemia patients with healthy kidney tissue only l / 3 of the normal person, and then a little damaged, it will be a lethal uremic syndrome. Especially, peanuts contain many non-essential amino acids, not high-quality protein, a large meal of peanuts, of course, will be misplaced. So, azotemia patients do not eat protein at all, is not better? That also can not, protein into the body, digestion, absorption, will be transformed into amino acids, and essential amino acids are essential for human metabolism, is to maintain life can not be missing. Therefore, medical doctors believe that: lighter azotemia patients, the daily intake of protein about 40 grams of left to have appropriate, should choose to eat high-quality protein, that is, protein rich in essential amino acids of the body; preferred for eggs and dairy food, an egg contains about 6 grams of protein, a bowl of milk (about 200 ml) contains about 6 grams of protein, 50 grams of lean meat contains about 8 grams of protein, chicken contains slightly more protein than lean meat, and fish slightly less. As mentioned above, it is necessary to limit the intake of protein, but the human body needs a certain amount of daily caloric energy to live, protein eating less, it must eat more carbohydrates (such as sucrose, grape enamel) and fat (preferably eating vegetable oil) to supplement, such as the patient’s appetite is not good, you can give the Chinese medicine Er Chen Tang (half summer, Poria, Chen Pi, licorice) and add fried grain buds, Shen Qu and other demulcent drugs, to promote appetite . In addition, attention should be paid to the supply of food rich in B vitamins and vitamin C-rich food in the food. As azotemia worsens, the amount of protein consumed should also be reduced. However, if the daily protein intake is too low for a long time (less than 20 grams), the patient will not be able to live, and in this case, dialysis therapy will be performed. However, dialysis therapy is a last resort because it has many side effects and is expensive. The best course of action is to follow a strict diet to delay the onset of the need for dialysis.
