Sagging in the middle of the face (cheeks) refers to the sagging of the cheek area below the eye lobes, the appearance of flabby cheeks, and the deepening of the nasolabial folds. It is caused by the aging of the facial tissues from the surface to the interior (subcutaneous skin, deep fascial structures, etc.), which become less elastic, and then stretch and shift under gravity, resulting in sagging. The principle of correcting sagging cheeks is to tighten and maximize the retention of the sagging structures. Since most of the facial tissues are soft, it is most challenging to maintain the lifting and tightening. The only way to maximize the correction of mid-face ptosis is to tighten the denser fascial structures in the deep side to make the foundation of the face firm, and then lift the loose subcutaneous tissues of the skin and allow them to heal and adhere to each other in an elevated position. Various suspension techniques (fascia, serrated wires, etc.) are characterized by minimal invasiveness, requiring only a few needle holes to bury various suspension materials under the skin of the face and then suspend them upward for fixation. All of these methods utilize the elasticity of the already loose tissue to lift the sagging facial tissues, with the force of the lift being borne entirely on a few fixed points. It is important to know that the soft tissues in any part of the human body will grow in response to pulling, just as the belly of a pregnant woman will become loose after a long period of expansion, and overweight earrings will open up the ear holes, the facial skin tissues, if lifted tightly by a few suspension points, will soon be loosened by these points, and this is the reason that all minimally invasive suspension methods to lift the face are short-lived (less than 3 months). Even if the suspension wires are bought into the face in a variety of curved shapes, the effect will not be long-lasting and will cause other problems due to the retention of foreign bodies. Therefore, these methods should be chosen with caution.
