Constipation is not a disease, but a complex clinical symptom caused by the pathological process of multiple diseases. The main clinical manifestations are difficulty in defecation or poor defecation, reduced frequency of stool or little bowel movement, feeling of incomplete defecation, accompanied by abdominal pain, abdominal distension, nausea, loss of appetite and mental symptoms (irritability, anxiety, insomnia, depression), etc. Constipation is a common clinical symptom and has become one of the most important factors affecting the quality of life of modern people. The following mainly discusses the comprehensive treatment of chronic constipation. 1, the general treatment mainly includes including diet therapy and psychotherapy. ① Diet therapy: this method is the basic method of treating constipation, including drinking more water, eating more fresh fruits and vegetables and foods rich in coarse fiber, etc. ②Create good defecation habits: First of all, you should give up the existing bad habits, such as artificially suppressing the desire to defecate, reading books and newspapers during defecation, causing excessive defecation time, and excessive forceful defecation. On this basis, the normal conditioned reflexes of defecation, i.e. the rising reflex in the morning and the gastrocolonic reflex after eating, should be used to defecate. ③ Exercise therapy: defecation needs to raise intra-abdominal pressure, mainly relying on the strength of the diaphragm and abdominal muscles, so often deep breathing exercises to enhance the strength of the abdominal muscles, which is conducive to the discharge of feces. In addition, exercise can stimulate colon peristalsis, accelerate the propulsion of intestinal contents, which is conducive to defecation. For some patients with outlet obstruction type constipation, long-term adherence to the thoracic knee position anal lifting exercise is conducive to strengthening the strength of the pelvic floor muscles and enhancing their coordinated motility, which can greatly reduce the symptoms and even heal, especially for patients with rectal prolapse, the effect is more obvious. ④Psychotherapy: to eliminate the patient’s nervousness, anxiety, depression and other mental abnormalities. Psychotherapy has a positive therapeutic effect on patients with severe constipation. Population surveys and clinical observation studies show that a certain intensity of psychological or physical stress can disrupt normal gastrointestinal motility, so full attention should be paid to psychotherapy in the treatment of constipation. 2, drug therapy For more serious constipation, laxatives and gastrointestinal prokinetic drugs can be applied as appropriate, the choice of drugs should be based on the principle of minimizing toxic side effects and drug dependence, avoid long-term application or abuse of stimulant laxatives. In the selection of constipation treatment drugs, only consider its efficacy and onset of action time is no longer in line with the requirements of modern treatment, should also consider whether the drug can be used for a long time, how safe it is, and whether the patient has good tolerance of the drug. 3.Biofeedback therapy Biofeedback is a kind of bio-behavioral therapy, which transforms some physiological and pathological activities that are not easily perceived by the human body into easily perceived information such as sound and images through electronic engineering technology, so that patients can adjust their physiological activities according to their observed information of their own physiological activities to achieve the purpose of treating diseases. Including myoelectric biofeedback system and pressure biofeedback system, the purpose of biofeedback therapy is to make the trainees “change themselves”. 4.Surgical treatment Through the above treatment, the majority of constipation patients can be cured or improve the symptoms, but some patients with persistent constipation eventually need surgery. For example, slow transit type constipation, after the determination of anorectal dynamics, and strict conservative treatment is ineffective, has seriously affected the patient’s quality of life, can be considered for partial colectomy, subtotal colectomy, clinical results are satisfactory. For constipation with outlet obstruction, endorectal mucosal fixation, rectal protrusion repair, partial resection of the puborectal muscle and other surgical treatments are feasible, and the results are also more satisfactory. At present, there are more than 10 types of surgical procedures for constipation. However, we must be clear that constipation is often two or even a variety of diseases or symptoms mixed together in the syndrome, must strictly grasp the indications for surgery, should be aimed at relieving the patient’s symptoms.
