Causes and differences of morning back pain, evening back pain and midnight back pain

  Low back pain is the most common clinical condition, with about 80% of people experiencing low back pain in their lifetime. For many people, when they run into low back pain, they often stick a piece of dog plaster on themselves or look for a massage therapist for massage. Professor Shen Huiyong, director of orthopedics and president of Sun Yat-sen Memorial Hospital of Sun Yat-sen University, said that low back pain refers to pain in the lower back, lumbosacral, sacroiliac and other places, and its causes are very complex with diverse clinical manifestations. However, different low back pain has different attack times, and different attack times can suggest different causes, so the people can pay close attention to it to help the doctor’s differential diagnosis greatly.  The most common cause of low back pain is lumbar disc herniation. As most people stand upright during the daytime work, the weight of the body can flatten the intervertebral disc, and if it protrudes to the back side, it will squeeze the nerve roots immediately adjacent to it, causing lumbago combined with soreness, numbness and pain on the back side of the lower limbs. The lumbar region is located in the lower part of the trunk and naturally bears the most weight, plus the lumbar region is the most active part of the trunk, the longer the day’s work, the more prominent the lumbar disc is, so the more lumbar pain is exacerbated. After a night’s rest, the intervertebral disc is slightly reset, the pressure on the compressed nerve is reduced, and the lumbar pain is relieved, so patients with herniated lumbar discs tend to have less lumbar pain in the morning, or even no pain at all, but the work starts after noon, and the more painful it is in the evening.  The pain caused by inflammation of tissues, such as ankylosing crestitis, tuberculosis or osteomyelitis, fibrositis, fasciitis, vasculitis, etc., is most painful when waking up in the morning, but after activity, the painful symptoms are reduced or disappear. Because there is no activity in the night, the waste materials produced by metabolism accumulate in the local tissues, stimulating the painful nerves and causing back pain, and after activity the blood circulation increases, taking these waste materials away, thus reducing the pain. Menopausal women may also suffer from low back pain due to plant nerve dysfunction, which is also characterized by being heavy in the morning after waking up and relieved after activity.  If you suddenly wake up from a dream with pain in the middle of the night or late at night, then such back pain may indicate cancer. Cancer may be primary or metastatic. Benign bone tumors usually do not cause pain. Bone cancer pain is the most intense of all cancer pains. It is characterized by resting pain, the quieter it is, the more painful it is. When activity is opened, the pain is reduced instead. It is presumed that it is because the activity causes the tumor factor to dissipate and no longer compress the nerves. In addition, bone cancer has another characteristic, that is, the pain will intensify if you lightly tap on the painful area, which is the opposite of the symptoms of lumbar muscle strain and lumbar disc herniation that feel more comfortable after massage and tapping.  The back pain caused by other organ diseases also needs to be taken seriously by the people. Urinary system infections, kidney lesions, female gynecological inflammation, pelvic tumors, etc. can cause low back pain, while, duodenal ulcers can sometimes cause radioactive pain in the lower back. These low back pains do not increase with activity and do not disappear with rest, and there is no time difference. There are times when the patient lies in bed and turns around and the low back pain is relieved, but for sure, the bell must be untied and these low back pains will only be cured if the disease of the organ itself is resolved.
