Achalasia of cardia (AC) is a primary esophageal motility disorder, clinically manifested as dysphagia, chest pain, food reflux and heartburn, which seriously affects the quality of life of patients. Currently, the main treatments for this disease are: 1) pharmacological treatment, such as calcium antagonists, nitrates, etc.; 2) surgical treatment, such as sphincter of Heller myotomy, etc.; 3) endoscopic treatment, such as esophageal dilatation, esophageal stent placement, botulinum toxin injection, transoral endoscopic myotomy, etc. Per-oral endoscopic myotomy (POEM) is a new treatment for pancreatic flaccidity, which is a natural endoscopic procedure. The main surgical steps include: ① esophageal mucosal layer incision; ② separation of submucosal layer; ③ establishment of submucosal “tunnel”; ④ incision of circular muscle under direct view of gastroscope; ⑤ metal titanium clip to close the mucosal layer incision. The mucosal layer incision is closed. Compared with traditional treatment methods, this technique has the advantages of less trauma, faster recovery, safer operation, and significant recent efficacy.
