Symptoms and treatment of anal fissures.

  Anal fissure is also a common disease, often asked by patients and friends, I now summarize its symptoms and treatment as follows: Anal fissure is due to fatigue, alcohol, spicy food and other factors, resulting in dry and hard stool, excessive force during defecation, damage to the skin of the anal canal, repeated damage to the fissure deep and full skin, the formation of chronic infected ulcers. Its clinical symptoms are as follows: ① pain: dry hard stool directly tears the skin of the anal canal or holds open the fissure during defecation, forming severe pain, which is relieved briefly after defecation, and after a few minutes due to reflex spasm of the sphincter, causing a longer period of intense pain, which we call periodic pain. ②Constipation: Because of the patient’s fear of severe pain during defecation, the stool stays longer in the rectum, water is completely absorbed, and the stool becomes more dry and hard, which in turn will aggravate the fissure and pain, forming a vicious cycle over time. ③ bleeding: dry and hard stool, tearing the fissure, can cause fissure bleeding. (iv) Systemic symptoms: Anal eczema and anal scratching are caused by secretions from the fissure ulcer surface or subcutaneous fistula, or secretions from the anal glands, which stimulate the skin of the anal margin.  The treatment of anal fissures varies clinically depending on fresh and old anal fissures. Fresh anal fissures can be healed by non-surgical treatment, and attention should be paid to diet, softening stools, eating more vegetables and fruits, abstaining from spicy food, and not drinking alcohol. Old fissures are often associated with anal papilloma, sentinel hemorrhoids, subterranean fistula, anal cryptitis, etc. and require surgery. The specific surgical method depends on the patient’s condition, but the principle of minimally invasive and painless treatment should be adopted. At the same time, concomitant papilloma, sentinel hemorrhoid, subterranean fistula, cryptitis, etc. should be treated together.
