Pediatric appendicitis is one of the most common acute abdominal conditions in children. Many young parents have little knowledge about it, causing many pediatric appendicitis to be misdiagnosed, and by the time it is identified many of them become periappendiceal abscesses, or the appendix is already perforated before surgery is performed, missing the best time for surgery and causing great harm to the pediatric patient, or causing various complications after surgery: wound infection, abdominal residual abscesses, etc., with long treatment time and poor results, and even leaving permanent scars. For the causes of appendicitis are mainly diarrhea, respiratory infections, gastroenteritis, etc., which are mostly not characteristic, I will not discuss them too much here, and they are not more relevant. What parents need to pay attention to is the manifestation of the disease, parents should reacquaint themselves with the disease, the characteristics of pediatric appendicitis are as follows: 1. Most of them start with fever or abdominal pain, sometimes with the manifestation of respiratory infection such as mild cough and runny nose, at the very beginning many parents or clinics will diagnose with respiratory infection and gastroenteritis; 2. Abdominal pain will be vomited in the early stage, after that vomiting will be relieved or disappear (clinical experience), but abdominal pain and The abdominal pain is not fixed in the early stage, mostly around the umbilicus, but later shifts to the right lower abdomen or lower abdomen; 4, a routine blood test will reveal high white blood cells and elevated neutrophils (this is very important), simple respiratory infection and gastroenteritis should not have such a high blood picture. With these experiences, if parents have a high suspicion that their child has appendicitis, they should ask the doctor to do an examination of abdominal signs (especially important), flexing the lower extremities in a lying position and palpating the abdomen of the child, if there is pressure pain and localized abdominal muscle tension in the right lower abdomen most likely the diagnosis of the disease. Especially if the doctor is an internist many will ignore this point, if you do not feel comfortable to find a surgeon to do this sign. Our famous pediatric surgeon summarizes the key points in the diagnosis of pediatric appendicitis: metastatic right lower abdominal pain and pressure pain at a fixed point in the right lower abdomen. Once the disease is diagnosed, the patient should be seen at the nearest hospital and operated on in time, if the adult hospital cannot be anesthetized it needs to be transferred to the general surgery department of the children’s hospital in time. Early surgery for this disease has very few complications and heals particularly well. The Second Department of General Surgery of our hospital has authoritative standards and advantages in diagnosing pediatric acute appendicitis. In particular, the laparoscopic technique for carrying out appendicitis is very mature. It has the advantages of small scars (3 wounds less than 1 cm, one of which is located at the umbilicus), fast postoperative recovery, little trauma, few complications, and the possibility of exploring the abdominal cavity. Laparoscopic appendectomy can also be performed directly for chronic appendicitis. Minimally invasive laparoscopic surgery is also feasible after recovery from past misdiagnosed appendicitis. A special reminder is that if appendicitis is diagnosed too late and forms an abscess, there is no way to perform laparoscopic surgery, so parents need to know more about the characteristics of pediatric appendicitis, early diagnosis, early treatment and early recovery. Once appendicitis perforates or forms a periappendiceal abscess with heavy infection or even death from infectious shock, treatment is very difficult, long (at least two weeks), costly and prone to long-term complications. The above is my own knowledge of pediatric appendicitis, there are incomplete places, mainly to remind all young mothers and fathers do not let this common disease to their babies bring a variety of pain.
