Individualized treatment of elderly and elderly lung cancer patients with radiofrequency knife

  The patient was a male, 80 years old, in good health, without hypertension and diabetes mellitus. 2012, the patient felt pain in the right side of the back, without cough, occasionally with blood in the sputum, and a lung CT examination revealed a right upper lung tumor, and further whole-body PET-CT confirmed a malignant tumor in the right upper lung. Since the tumor was located at the lung apex, puncture biopsy was very difficult. Radio-wave knife treatment was chosen as the best treatment option. The patient had occasional right shoulder pain after the radio wave knife treatment, and the shoulder and back pain disappeared 3 months after the treatment. The lung CT was repeated and confirmed that the tumor had shrunk by more than 60%.  Images of the radio wave knife treatment plan. For right lung cancer, the tumor was located at the tip of the right lung. In order to reduce the brachial plexus nerve injury, we implemented a treatment plan of 4 treatments to increase the dose in most of the tumor area by reducing the isodose curve that wraps around the tumor. That is, the tumor received a higher dose within the tumor.       Three months after the radio wave knife treatment, a repeat lung CT showed that: the lung cancer had shrunk by more than 60%.  The patient lived a normal life with no discomfort. Two years after the treatment, the lung CT and PET-CT were repeated at my request. The results of the review suggested that the right lung apical lung cancer treated with radio-wave knife was well controlled and the tumor was not metabolized (evidence of tumor death). However, a new tumor appeared in the middle lobe of the right lung. We treated again with radio-wave knife.  2 years after radio-wave knife treatment, PET-CT showed tumor death. However, a new tumor appeared in the middle lobe of the right lung.  Conclusion and experience: It is extremely important to choose an individualized treatment plan for elderly and elderly patients, who have lung cancer. This patient was treated locally with the radio-wave knife for the tumor only, and the patient had no pain and a high quality of life.
