Abstract: Mr. Zhang, 76 years old, visited our hospital with dizziness and lightheadedness for 1 day, complaining of the presence of prostatic hyperplasia, a history of bilateral carotid atherosclerosis, and a history of coronary atherosclerotic heart disease. I found excessive high pressure and increased pulse pressure difference through physical examination, and then found aortic calcification through X-ray, which could be diagnosed as aortic atherosclerosis. After taking Irbesartan tablets, furosemide tablets, benzofibrate dispersible tablets and aspirin enteric soluble tablets for treatment, the condition was stabilized.
Basic information】Male, 76 years old
Disease Type】Aortic atherosclerosis
Hospital】The First Affiliated Hospital of Xi’an Jiaotong University
Date of consultation】March 2021
Treatment plan】Medication (Irbesartan tablets, furosemide tablets, benzofibrate dispersible tablets, aspirin enteric soluble tablets)
Treatment period】Hospitalization for 5 days, discharged from hospital for monthly review
Treatment effect] The physical discomfort gradually disappeared, blood lipids and blood pressure were stabilized, and the disease was controlled.
I. Initial consultation
Mr. Zhang, 76 years old, felt dizzy and lightheaded for the last day, but had no other physical discomfort, so he came to our hospital for a clear diagnosis. Mr. Zhang complained that he had a history of prostatic hyperplasia, bilateral carotid atherosclerosis, and coronary atherosclerotic heart disease. This time, the physical examination revealed high systolic blood pressure and increased pulse pressure difference, which was considered to have reduced aortic elasticity. This is generally due to the calcification of local plaque after aortic atherosclerosis, and elevated total cholesterol level can be found through blood biochemical examination, which is considered to be caused by lipid deposition on the arterial wall. He was admitted to our hospital for treatment.
II. Treatment process
After the diagnosis, Mr. Zhang explained his condition and the drug treatment plan to him, and he agreed. Because hypertension can cause damage to the endothelial cells of the aortic vessels, as the disease progresses, atherosclerotic lesions will appear in the arteries, so it is necessary to take Irbesartan tablets to dilate the blood vessels and promote the fall of blood pressure, while furosemide tablets can be used to diuretic, which also has the effect of lowering blood pressure. At the same time, blood lipids are regulated by benzofibrate dispersible tablets to avoid the aggravation of atherosclerotic plaque. Because of the elevated blood lipids and increased blood viscosity, aspirin enteric-coated tablets are considered to inhibit platelet aggregation and prevent the formation of arterial thrombosis. At the same time, bilateral carotid atherosclerosis and coronary atherosclerotic heart disease will also be improved. As for prostatic hyperplasia, it needs to be treated in our department and then transferred to the department for further treatment.
III. Treatment effect
After 1 month of treatment with Irbesartan tablets, Furosemide tablets, Benzafibrate dispersible tablets and Aspirin enteric soluble tablets, Mr. Zhang’s complaints of dizziness and lightheadedness disappeared, and the high pressure was found to be controlled within the normal range through physical examination, and the pulse pressure difference was normal. Then it was clear through X-ray examination that aortic atherosclerosis was not further aggravated, and the total cholesterol level could be found to be back within the normal range through blood biochemical examination, considering that the lipid-lowering medication was taking effect. After that, the blood pressure and lipid condition were good and stable when we came to check every month.
IV. Precautions
I am glad that Mr. Zhang’s condition is stable after receiving treatment, but he needs to take medication for a long time after suffering from aortic atherosclerosis, and he should not adjust the dosage or duration of medication by himself to avoid affecting the therapeutic effect of medication. If any symptoms like nausea, vomiting or other adverse drug reactions occur while receiving treatment, it is recommended to stop medication and seek medical attention in time. Pay attention to eat more food rich in minerals, vitamins and other nutrients, which can help to nourish the body. And pay attention to go out for a walk and exercise every day, which can improve the energy and maintain a good mental state, and also help to control blood lipids. Blood pressure needs to be measured after using furosemide tablets and irbesartan tablets to clarify the effect of lowering blood pressure as well as to prevent hypotension.
V. Personal insight
Mr. Zhang suffers from aortic atherosclerosis, coronary atherosclerotic heart disease, bilateral carotid atherosclerosis cardiovascular disease, considered mainly due to elevated blood lipids and blood pressure, so the usual lipid regulation, blood pressure treatment need to pay special attention to strictly adhere to the diet, exercise care, so as to avoid the condition continues to be serious. Since Mr. Zhang was stabilized with medication in time, no serious accident occurred.
