If liver function is unstable, pregnancy is generally not recommended because of pathophysiological changes during pregnancy, which can increase the burden on the liver and thus aggravate abnormal liver function. Pregnancy is still possible in HBV patients with stable liver function in several cases. The first is in HBV-infected patients in the immune tolerance period, where such patients have active viral replication and positive e antigen (a soluble antigen of hepatitis B), but normal liver function and no antiviral indication, but the possibility of mother-to-child transmission is high, and after blockade therapy, about 85% of offspring will not have chronic hepatitis B infection occur, and there are also reports show that this transmission is associated with maternal HBV DNA titers; some investigators have applied drugs to reduce the amount of HBV DNA in the first 3 months of pregnancy and applied high potency hepatitis B immunoglobulin and hepatitis B vaccine to block mother-to-child transmission after the birth of the infant, with satisfactory results. The second is patients in antiviral treatment, this kind of patients have entered the immune clearance period, after a period of antiviral treatment, HBV DNA has turned negative, but not yet the disappearance of e antigen, liver function is stable. This kind of patients can not stop the drug temporarily, otherwise it will cause the virus to rebound, and generally do not advocate pregnancy, because pregnancy during the drug may have some impact on fetal development; if the patient must request pregnancy, some studies have shown that lamivudine or tebivudine (two anti-HBV drugs) continued application is relatively safe, patients can choose. The third type is a carrier with stable disease, showing negative HBV DNA, disappearance of e antigen, appearance of e antibody, and normal liver function; such patients do not need to take antiviral drugs and can become pregnant, and after the baby is born with hepatitis B vaccine, the probability of mother-to-child transmission is almost zero.
